This Self-Assessment Booklet provides you with an opportunity to take inventory of your past experiential learning in classroom,
laboratory and clinical settings. Completion of the document is intended to assist you to identify priority learning needs that may be
met in this final nursing practice course. It is anticipated that you will share your completed Self-Assessment Booklet with both your
Clinical Instructor and Preceptor at the beginning of your clinical placement.

Within PART A, you are provided with an opportunity to identify the settings in which you completed your clinical learning. Your
preceptor will be able to determine where the learning opportunities available in the practice setting may align with your identified
learning needs. It is not the intent to “collect” checkmarks for each item identified in PART B of the assessment, but rather to
stimulate dialogue as to how you may continue learning in your current clinical placement.
