PARENTAL CONSENT FOR STUDENT INTERNSHIP

I hereby willingly and voluntarily give consent to (Name of Student Intern) a Bachelor of
Science in Computer Engineering student from College of Engineering, Morong Campus to
undergo 300 hours of student internship at (Host Training Establishment) from (inclusive
dates).

I have considered the benefits that my child will derive from undergoing virtual/online
internship, with the understanding that due care and precaution will be observed to ensure the
comfort and safety of the intern/student to this activity and that I shall not hold the parties
responsible for any untoward incident that may happen beyond their control.
