2.1. DESCRIPTION OF ACTIVITY:

The Lift Directive (2014/33 / EU) published in the Official Gazette dated 29 June 2016 and numbered 29757, within the
framework of Section 4, Article 19 (a) of the 2014/33 / EU Lift Directive IV. For the purpose of issuing an EU Type
Examination certificate in accordance with subparagraph b of the Annex, the inspection and test procedures of the
applicant's Electric Lift were carried out in accordance with the 2014/33 / EU Lift Directive Annex-I Basic Health and
Safety Rules and TS EN 81-20 and TS EN 81-50 standards.

2.9. FEATURES OF THE WELL: Concrete Wall Construction Well, Steel Construction Well.

2.10. INSPECTION AND MAINTENANCE CONDITIONS : Ordinary inspection and maintenance activities shall be performed
from machinery spaces where machinery is located in a machinery room. If the machinery is located inside the well
maintenance activities shall be performed from the top of the car and car is blocked by blocking devices which is connect to
the guide rails.

SECTION 3: ASSESMENT ACTIVITIES

Evaluation activities were carried out in accordance with sub-clause B of Annex IV of 2014/33 / EU Lift Directive, examination
of the technical file and test, test and inspection procedures were carried out on the sample lifts representing the types
specified in the technical file.

3.5. EVALUATION:
The evaluation activity was carried out by examining the technical files submitted by the applicant and performing the
inspection and test procedures of the model elevator assembled. The evaluation was carried out at the applicant's facilities,
in the installed elevator site and by the technical expert from D KARE head office with the participation of the applicant
representative Şamil ÇAHAL.

in the address stated above meets the essential requirements of annex I of 2014/33/EU Lift Directive. This report is related to
