AFFIDAVIT
I, MARK KEVIN CALAPILLO of legal age and a resident of Barangay
Tumcon Ilaud, Pototan, Iloilo after having been duly sworn an oath in accordance with
law, hereby voluntarily depose and say:
That I am the incumbent secretary of Pototan Farmers Multi-Purpose Cooperative
in Matias Yusay Street, Pototan, Iloilo requesting for loan for its livelihood projects from
the Iloilo Provincial Government in the amount of Ten Million Pesos (P 10,000,000);
That based on my personal knowledge, none of the incorporators, organizers,
directors or officials of Pototan Farmers Multi-Purpose Cooperative is an agent of or
related by consanguinity or affinity up to the fourth civil degree of the officials of the
Iloilo Provincial Government authorized to process and/or approve the proposal, the
Memorandum of Agreement and the release of funds.
That I execute this affidavit to attest to the truth of the foregoing and for whatever
other lawful purpose this may serve.
