Overview
Pulmonary embolism and deep vein thrombosis
(DVT) are important causes of morbidity and
mortality, hard to recognize in medical practice
(PE diagnosis in the life of the patient is rare -
10-30%)
The incidence of PE is 20,000 cases / year in
Romania, and in the US 500,000 / year.
Actual DVT and EP are seen as a common
entity - pulmonary thromboembolism, given
causal relationship and common treatment

The European Diagnostic and Treatment Guide of THP
divides it into 3 categories:
Massive PTE - cases with cardiogenic shock and / or
hypotension (systolic TA <90mmHg)
Underlying PTE - Widened and Hypokinetic VD on
Echocardiographic Examination
Unsolicited APE - without alteration of ultrasound VD
without significant haemodynamic disturbances
Etiology
Pulmonary thromboembolism is the consequence of
migration in the pulmonary circulation of emboli
fragments from a deep vein thrombosis
The most common thrombi come from lower limbs (70-
80%),
the more proximal the thrombosis, the higher the risk of
thromboembolism
In 10-15%, the thrombi come from the upper cavity
system, more often secondary to venous catheterization
or mediastinal syndromes.
Sometimes the thrombi come from the inferior vena
cava, a situation that is hard to detect in the absence of
invasive investigations.
Etiologie
Clinic, TEP este diagnosticat în doar 30% din cazuri. În
70-90% din cazuri, locul de unde a plecat trombul este
decelat cu ocazia examenului necroptic.
În unele cazuri, trombul care a obstruat vena
embolizează în totalitate şi nu mai este depistată sursa
emboliei.
Majoritatea trombilor sunt suficient de mari pentru a
obstrua o ramură principală a arterelor pulmonare.
Rareori, în TEP masive este obstruată o arteră
pulmonară principală, sau chiar amblele (tromboză în
şa).

Anticoagulantele orale:
Warfarina, acenocumarol
Metodă rar folosită, la bolnavi cu risc
foarte crescut de TVP,şi TEP
Minidoze de warfarină (1 mg/zi, cu INR –
1,5-2)
Profilaxia TEP
Prevenirea trombembolismului pulmonar
recurent după TVP se face prin:
Administrarea de anticoagulante orale pe
termen indefinit
Plasarea de filtre în VCI pentru pacienţii cu
contraindicaţii la ACO, la care tratamentul cu
ACO este insuficient sau s-a soldat cu accidente
hemoragice majore
Tratamentul curativ
Cuprinde:
Tratament anticoagulant cu:
Heparină nefracţionată
Heparină fracţionată
Anticoagulante orale
Terapia fibrinolitică
Embolectomia
Terapia stării de şoc, a hipovolemiei şi
decompensării drepte
Tratamentul anticoagulant
