Prepare the required documents a week prior to the class week

a. Class Program (see Annex 1A to 1D for sample Class Program)


b. Teachers’ Schedule (see Annex 2A and 2B for sample Teacher’s Schedule)
c. Weekly home learning plan (WHLP). From intermediate level up to grade 12, the
subject teacher shall submit to the adviser the contents of the WHLP of their
respective learning area which the latter will then consolidate, they may use google

01 docs for easy consolidation. (See Annex 3 for the sample WHLP)
d. Lesson Guide with Reflection (the reflection part shall be done per subject per
section) see Annex 4 for the Lesson Guide template
e. Learners’ Individual Monitoring Plan (see Annex 5 for the template)
f. teachers contact number/s
g. Directory of learners with parents/guardians contact information

Submit the said documents to your respective head teacher/principal/ subject

Scan modules ( as regard completeness of page, fact checking,

Distribute/provide parents/guardians with a copy of the following


documents:
a. weekly home learning plan (adviser)
b. Class Program
04 c. Teachers’ Schedule
d. complete set of learning modules
e. Learning /activity sheets

Communicate/ check learners if they have received the modules
appropriate for the week.

Perform the Teacher’s Tasks as stated in their schedule

a. Conduct of synchronous meetings. Synchronous meetings may be aided by the


use of google meet, zoom, text messaging, video call, or any resources available to
the learners. The following activities can be done during synchronous meeting :

06 i. Explain rules/proper behavior on modular distance learning, weekly home


learning plan, teachers weekly schedule and contact information
ii. clarification on lessons/tasks
iii. Review of the previous lesson
vi. Validation of learning/ recitation
b. Doing follow-ups to learners during asynchronous learning, giving more
attention to learners who are struggling.
c. Addressing queries during consultation hours

(For learners who cannot attend synchronous meeting) Record/log all text

07 messages and call logs relative to the conduct of MDL particularly the queries,
issues and concerns of the learners/parents

Receive learners’ outputs and provide feedback thereof. Accomplish the Reflection

08 part of the lesson guide and the Learners’ Individual Monitoring Template (if
applicable)

Submit accomplished lesson guide/reflection to the respective head
teacher/principal/ subject group head
