- SF10ES: ECCD Checklist (kinder) /Certificate or equivalent document

• Class Adviser of other grade levels will update using the same/original SF10
• Every End of School Year ONLY (not every end of the quarter)
• Kindergarten will only use the ECCD checklist instead of SF10/Form 137

B. Transfer and Safekeeping


• School where the learner completed Grade 6 and Grade 10 will
keep the Original Copy of SF10.
– Duplicate Copy will be kept by the originating school in case of transfer to
other school.
– Use separate grading box if transferred within the School Year. Copy
existing grades for previous quarter/s to complete the current grading box.

Deped Order 69, 2016 introduced the SHS


Form 137. It is a collaboration with BHROD-
SED, EMISD-PS, USD-ICTS & BLD – SHS
Task Force
• Same norm and protocol with SF 10-ES & JHS
• General Average from the 2 semesters in a SY
• Certified True Copy will be provided to SDO
once graduated.

• Documentary Stamp Tax (DST) in SF 10? Under
review but right now, it is not included in the list of
requirement stipulated in DO 54, 2016 and DO3
2018. The Transaction is just within Deped
Offices/Schools.

planning.

● In this time of pandemic, the LIS record of migration of


learners from private schools to public schools is one of
the indicators being monitored to provide
support/assistance for both sectors of education.

ENROLLMENT HISTORY

● The LIS Enrollment History record is one of the measure


to validate authenticity of the learner’s enrollment claim.

● In time of calamity/disaster such as the Yolanda Storm


Surge, Marawi Siege and Taal Volcano Eruption, the LIS
Enrollment Data was used to facilitate and validate
enrollment of affected learners who lost/destroyed paper-
based record of enrollment credential.

● Since the introduction of LIS, a considerable amount of
clerical works of our teachers were drastically reduced
since teachers are no longer need to create a new record
of learner who transferred in from another school.

● Information being shared thru this LIS facility is


considered as tampered proof as only authorized
personnel is allowed for access

How The Tracking System Works
The tracking system is highly dependent on the “cooperation” of
both originating and receiving schools or CLCs.

a. The receiving school (School 2) initiates the request upon enrollment


of learner in LIS. The system will notify the originating school and
validate if the proposed transfer is legitimate.

b. The originating school (school 1) is expected to response within thirty


(30) days upon received of the electronic notification. The receiving
school must decide either confirm or decline the requested transfer.

1. Learner is still with us (Mistaken Identity)
2. Learner is attending classes in another School. (Attending in a
school different from the school that made the request)
3. Deceased

CONFIRMING TRANSFER REQUEST

By confirming legitimacy of a requested transfer, the originating


school indicates the date of last attendance of learner in their school
and the date when the credential document sent thru courier or in
any form as possible and safe except hand it over to s the parent or
party with interest. There is also an information box that the
originating school can use to provide additional information as
needed.

How The Tracking System Works

What if the learner has unsettled financial obligations to the private


school? Is the confirmation of transfer still applicable?

Yes, since the Tracking System requests only the confirmation of


“Physical Transfer” of learner and it has nothing to do with the clearance
of learner (e.g any debt in the school). The private school must tag the
learner as “with financial obligation” upon confirming the physical transfer
so that the system automatically marks the learner as “Temporarily
Enrolled”.

receiving and originating school” Deped Order 54, 2016 Section V,

Challenges of Tracking System
● Some schools take some time in enrolling a transferred learner in the
system. They want first to observe the attendance performance so that if
the learner stops attending classes, it will not be counted as dropped out
in their school. As a resul, the originating school has no idea or
information about the current school of their former learner.

● Some private schools intentionally delay confirmation of transfer to force


parent to settle unpaid financial obligation.

● Some schools do not check regularly the transfer requests pending in


their account.

● With the increasing demand for learner’s performance,
the LIS is highly considering expansion of data being
collected and has started the technical assessment and
system capacity study.

● Initial consultation with other key stakeholders and


partners is ongoing to further improve the LIS Tracking
System.
