When:20:30 pm, 28th MARCH 2009
Where: Paper building construction site PCC
ROOM,
Who: Sterling electro team, having approx two
month of experience at this construction site.

 Description of Incident

• Around 20:30 pm while shifting of

panels in side the PCC room (manually)
the panel fall down, No casualty reported
as NEARMISS.

 Emergency Management
• Immediately stopped all the sterling electros
work & shared about the dangerous to all the
other contractors by assembling at emergency
assembling point next day.

 Communications

• As per the communications system management

was informed same day on the initial incident
description.
 Action Plans
• JSA job steps should be identified by work
supervisor, hazard identification should be done by
safety supervisor & approved by construction
safety team.
• JSA should be available & shared with working
crew before starting of the work.
• Daily work must be aligned with initiator, he is
accountable for related JSA is available &
followed.
• Before performing any task railing, platform & a
barricading to be installed.
• Train the construction safety team & contractors
on JSA development process.
