Permit to Work
Introduction

 The Permit to Work Procedure: It is a formal written procedure


that utilizes a document (Work Permit) to control work by means
of identifying potential hazards & accordingly incorporating
required precautionary measures.
 Objective: To ensure a coordinated, communicated and safe
system of work.
 All work carried out within facilities and around are properly
documented.

Work Permit
 The work permit is a document that specifies
precautions, identifies hazards & controls works,
which shall be carried out in hazardous or a
restricted area.
 The work permit requires authorization from the
respective Permit Issuer in whose area of control
the work is to be carried out.
 The work permit is also a means of
communication among various supervisors or their
representatives belonging to Operations,
Maintenance, Sponsoring Teams & Contract
personnel, who are involved in work preparation
and / or its execution.

Permit Applicant
Permit Applicant is an authorized person to raise / apply a permit for any
work within a restricted or hazardous area.

 He is member of sponsoring department who will initiate the work.


 He must have technical competence to consider and guard against
the potential hazards associated with work.

 He must have detailed knowledge of the Permit to Work Procedure.


 He is nominated in writing by the respective Team Leader.

Worksite Supervisor

The work site supervisor is a person authorized in writing by the


Sponsoring team to supervise the work:

 He must have a detailed knowledge of Permit to Work


System.

 All members of the work party adhere to safe working


practices and are fully conversant with the limitations,
restrictions and hazards involved.

 He does not leave the worksite during hot work, vessel entry,
radiography and excavation work.

Safety Rules / Procedures – Permit To Work

When a permit is required, we will:

1. Make sure we comply with the permit


conditions.
2. Ensure only permit –Trained persons
compile and approve permits.
3. Satisfy ourselves that all hazards have
been identified and risks mitigated.
4. Satisfy ourselves that all controls are in
place, including isolations. Isolations
integrity must be verified.

Safety Rules / Procedures – Permit To Work
When a permit is required, we will:
5. Consider interactions with other permits, or
simultaneous operations, cross referencing
these on the permit
6. Complete a tool box talk at the worksite,
involving all members of the team
7. Ensure we have read and fully understood
the requirements of the permit before
signing it
8. When work is completed or suspended,
ensure that the work site is left in a safe
condition

Work Permit Suspension
In case of a dangerous situation at the worksite or adjacent
facilities all work must stop immediately and shall not
recommence until the worksite is declared safe, and the permit
is subsequently endorsed by the permit issuer.

 The permit issuer, permit applicant and personnel from Fire


Team / HSE Team / HSE Group are authorized to stop the work
in the event of work permit violation, unsafe act or unsafe
condition.

 Any person can advise to stop the work if they observe any
hazard or potentially hazardous condition.

Problems Encountered on Permit Audits
 No flag man for equipment moving inside the plant.
 Inadequate barriers in place during crane operation
 Tool box talks are not conducted as a means of passing on
information.
 Housekeeping not carried out on a regular basis.
 Edges of excavations, trenches, not barricaded off properly.
 WSS are writing additional activates on the work permit,
without PMC knowledge.
 Contractors were working in numerous locations without a valid

permit.
 Work scope increases, PMC not notified and work continues.
 Permits are being submitted the same day as work to be
carried out, no 24 hour notice for PMC review.

REMEMBER

The issue of Work Permit does not, by itself, make a job safe. Safety
can only be achieved by those preparing the work & those carrying
out the work.
If you are unsure of any instruction on a Work Permit or in any doubt
regarding procedure, STOP & ask your immediate Supervisor for
guidance.
