ACTIVITES
• The SK Chairperson, through the SK Secretary, shall write a formal letter
to the Punong Barangay to:
• Inform PB on the conduct of Youth Profiling
• Seek Assistance and support from PB
• Request full support and cooperation of the residents

• SK Official shall conduct information campaigns or dissemination

• Announcement and posting the details in three (3) conspicuous and/or
posting in social media
• SK Official may divide and designate themselves, for faster data gathering

ACTUAL CONDUCT OF PROFILING AND
CONSOLIDATION
• Provide the respondent the Informed Consent (Annex 2) and Survey Form (Annex 3)
• Guide the Respondent in the Accomplishment
• Request the respondent to sign booth forms
• SK Secretary will consolidate the data using “Katipunan ng Kabataan Youth
Profile” (Annex 4)
• SK Chairperson shall provide the City Youth Development Office a copy of
Annex 4
• Observe the minimum health protocol during the conduct of KK Profiling
