THE HIP JOINT

• acetabulofemoral joint
• classified as an ‘enarthrodial’ (ball and socket)
• Acetabulum is formed by the three bones of the pelvis
(ischium, pubis, ilium)
• It provides a deep socket for the femoral head
• Like the glenoid labrum, there is an acetabular labrum
increasing the depth of the socket.

Movements:
-neutral position: slightly flected, external rotated, abducted position
during an inflamation in that joint we have the same position.

-anteversion (flexio) 120º


-retroversion (extensio) 12º
-abduction 45º
-adduction 15º
-external rotation 45º
-internal rotation 35º

Type:
classified as an ‘enarthrodial’ (ball and
socket) (art. cotolitica)

Ligaments:
- iliofemoral (350 kp)
- pubofemoral
- ischiofemoral

- zona orbicularis
- round ligament (ligamentum capitis )
Inner hip muscles:
-psoas major
-iliacus
(-psoas minor)
ILIOPSOAS MUSCLE
• Three muscles:
Iliacus
• Origin
• iliac fossa
• Insertion
• Lesser trochanter of the femur

Psoas Major and Minor


• Origin
• Transverse processes of L1-5
• Insertion
• Minor: pectineal line
• Major: lesser trochanter
• Action
• Hip flexion
Outer hip muscles,
I. Superficial layer:
-gluteus maximus et medius et
minimus
-tensor fasciae latae
GLUTEUS MAXIMUS MUSCLE
• Origin
• Posterior gluteal line of ilium
• Posterior iliac crest
• Lateral / posterior aspect of sacrum and cocyx
• Insertion
• iliotibial band and gluteal line of femur
• Action
• Hip extension
• External rotation
GLUTEUS MEDIUS MUSCLE
• Origin
• Lateral surface of the ilium
• Insertion
• Superior and lateral surface of the greater trochanter
• Action
• Abduction
• External rotation (posterior fibers)
• Internal rotation (anterior fibers)
GLUTEUS MINIMUS MUSCLE
• Origin
• Lateral surface of the ilium
• Insertion
• Superior/lateral surface of the greater trochanter
• Action
• Abduction about the hip
• Internal rotation
Outer hip muscles
I. Deep layer
-gluteus minimus
-piriformis (for. ischiadicum majus!)
-hiatus supra et infra piriformis
-gemellus superior
-obturator internus
-gemellus inferior
-quadtratus femoris
Flexors:
-sartorius
-quadriceps femoris /rectus

• Origin
• Superior ramus of pubis

• pectineal line of femur

ADDUCTOR LONGUS MUSCLE
• Origin
– Anterior pubis
• Insertion
– Middle third of the linea aspera
• Action
– Adduction
– external rotation
ADDUCTOR BREVIS MUSCLE
• Origin
– inferior ramus of the pubis
• Insertion
– pectineal line of the femur
– upper 1/3 of the linea aspera
• Action
– Adduction about the hip
ADDUCTOR MAGNUS MUSCLE
Origin
• inferior ramus of the pubis
• Insertion
• Whole length of the linea aspera
• Medial supracondylar ridge of femur
• adductor tubercle of femur
• Action
• Adduction
• External rotation
