Lockout
 Lockout is a technique used to
prevent the release of hazardous
energy or to prevent the energy
from release
 A padlock is used in the device
for the energy release that is in
the off or closed position

Tagging Authority

 Tagging Authority (Authorized Employee) - one who locks out


machines or equipment in order to perform the servicing or
maintenance on that machine or equipment. The tagging Authority
is responsible for identifying the isolation points and installing the
initial lock out using multi lock hasp and lock and entering all
details on the Safety Tagging Permit.

Permit Requestor (PR)

 The permit requestor is the person(s) who will be


undertaking the work activity on the plant/equipment
with stored energy.

 The permit requestor is responsible for ensuring


his/her lock, provided by the Tagging Authority is
applied to the point of isolation in accordance to the
Safety Tagging Permit

Responsible Maintenance/ Equipment Engineer

 The responsible maintenance/equipment engineer shall be


informed of the impending maintenance/repair work to be
carried out and shall not attempt to conduct any work until
permit is approved and personal lock has been applied to the
energy isolation point.

Affected Employee

 One whose job requires him/her to operate or use a machine or


equipment on which servicing or maintenance is being performed
under lockout or whose job requires him/her to work in such an
area in which LOTO is being performed.

Accidents
 About ten percent (10%) of all accidents in the workplace

out in containing dangerous and hazardous energy.

 Ensure equipment is safe to operate


 Safeguard all employees
 Last person to take off lock
 Follow safety tagging permit ( attachment B)

Safety Tagging Permit
 RA and JHA must be conducted for specific lockout and tag out
activity.
 Safety Permit must be in place before commencing with task.
 Tag numbers and individual initials accepting the permit must
sign on to the permit.
 After task is completed and the safety tagging is no longer
required, tags can be removed also the permit needs to be
cleared by entering the date and employee initials.
 If locks were installed, the initials also signify that the locks were
removed.

Temporarily Reactivating Equipment

 Remove unnecessary tools from the work area and


make sure everyone is clear of the equipment
 Remove lockout / tag out devices and re-energize the
system
 As soon as the energy is no longer needed, isolate
the equipment and re-apply lockout / tag out, using
the six step procedure.

 Never attempt lockout / tag out procedures unless you have


been trained and certified by project ES&H department
 Never loan or share your lock, combination, or key with anybody
else.
 Always make sure all Lock/tag devices are compatible with the
environment in which they will be used i.e. corrosive ,humid etc.
