Lack of situational awareness


Description of Deviations
This presentation originates from the statistical treatment of deviations related to the DOF Brasil fleet in the 2nd quarter of 2023
(Q2), we found that although we did not have systemic deviations, the type of deviation with greater quantity were related to the
LACK OF SITUATIONAL AWARENESS, according to the following graphs:
Campaign Objective

Present the results of the study of the deviations recorded in the fleet for the 2nd
quarter - 2023.
Reinforce awareness and situational awareness of employees' risks in the
workplace.
• Reinforce the importance of knowing the work environment so that we are more
aware of the risks to make safe decisions.
• Present the importance of classifying deviations and using the correct causes in
UNISEA.
What is situational awareness?

Correct perception of reality, coated with critical capacity of a given

situation, involving all its elements, in order to enable appropriate

decisions to be taken, within the available time, for the successful

fulfillment of the task.


Risk Perception / Risk Understanding
When you see a risk, it becomes
yours!

Everyone sees risk differently;


everyone thinks risk differently.
That's risk perception.

We measure the severity of the risk


differently. That is why it is important
to analyze the situation more
broadly.
You have to consider human, environmental and technological factors every day.

actions make a difference.

Life-Saving Rules

Following the Life-Saving Rules will keep you


and your colleagues safe.

Rules to help you identify hazards, manage


risks and put critical controls in place

Consult the Life-Saving Rules whenever


necessary and use them in your daily tasks.
Risk Assessment
Risk assessments must be carried out before
starting any work to reduce and manage risks.
Remember, if something changes on a task -

STOP,

if it is not safe to do so and perform another risk


assessment.

Keep the Risk Assessments up to date with the


tasks you are performing and pay attention to
completing the form correctly.
Permit to Work(PTW)
The Permit to Work (PTW) puts you in charge of
higher risk activities that require coordination and risk
control.

The Permit to Work (PTW) is up to you and your


colleagues to use it right.

Attention:
• Fill in all fields of the PT, TBT and RA form;
• Check if employees have up-to-date training
certificates;
• Attach the simulations, when applicable, and check
their correct completion.
Toolbox Talks (TBT)

Toolbox talks (TBT) are important because


they make employees aware of the dangers
involved with their next tasks.

This is your chance to talk and ask questions.

Helping you assess, plan and organize


activities before you start any task.

Pay attention to filling out the form correctly


and when finishing the task, register your
feedback/debrifing.
PPE

Protect yourself. PPE only works if you use it. It is


the last line of defense and is essential in all
workplaces.

Always consult the updated matrix of CAs and also


know the correct PPE for each activity (Matrices
available in DOCMAP).

Carry out the necessary PPE inspections and


record in the appropriate form..
Stop Work Authority

Raise your voice when you see a potential risk.

STOP!

Never remain silent.

Record your Stop Work Authority.

incidentes and repeating mistakes
You are an important messenger; your
feedback is used to make a safer workplace.

Use the safety observation system and register


at UNISEA.

Pay attention to filling in the necessary fields and


especially the correct causes.
Check Lists

Checklists support you in not forgetting the main security points to be observed before, during and
after tasks.

Know the Checklists / Verification Lists and permission documents that your activity requires.

• Specific navigation, approach and DP checklists;


• Tool, workplace and structural inspection checklists;
• Work at height and confined space checklists;
• Check lists at UNISEA for Drills, procedures training and realization of VCPs;
• Etc.
Security Barriers

The Behavior Based Safety Program (BBS) is


grounded in the importance of having consistent and
robust barriers in place in the workplace.

We believe that barriers play an important role in


preventing accidents and incidents in the workplace
by helping to make safe decisions.

1ª Security Barrier - Correct Prioritisation

• Using the time necessary to work safely;


• If a conflict arises between security-related issues and other important
business issues, such as production or costs, security must be prioritized
until the conflict is resolved;
• This means that if we are not sure that a task can be completed safely, we
should postpone the completion of this task until we have effective
security control.

2ª Security Barrier - Compliance

• With regard to loyalty to:


– Procedures;
– Requirements;
– Guidelines;
– decisions.
3ª Security Barrier - Open Dialogue

• Regarding openness and trust;


• We should feel free to discuss safety matters with any management
level, as well as with our co-workers;
• We must always insist on paying attention to safe work practices
and also demand actions that will contribute to the improvement of
these safe work practices.

4ª Security Barrier - Continuous Risk Assessment

• Just a simple, practical work habit;


• Just related to taking the necessary time to assess what types of
accidents can occur if something unexpected happens;
• Once accomplished, we can then use other security barriers to ensure
we don't put ourselves at risk.
5ª Security Barrier – Caring about each other

• It is about being responsible and intervening when we observe risky situations;


• Not only being responsible for ourselves, but also for our Partners when we identify that they put themselves at risk, including other
people around them;
• DOF Subsea HSE Policy:

– “Everyone has an obligation to stop or stop activities that put themselves or others at risk of accidents and must
ensure that appropriate control measures are implemented before continuing operations.”
