Dr. MARCI BOWERS says gender reassignment surgery isnt as traumatic as many fear. Complications, she says, are extremely rare, patients are in the hospital an average of only three nights and most are off pain medication within 48 hours.

what its like to be bald. Or like if youre selling sports cars but you drive a minivan. I know what the consumer is looking for but I think being a gynecologist is the most important. Because its a very visual surgery and very artistically based. If someone has a gall bladder out and there are no complications, nobody cares what it looked like but this surgery has such an artistic component, the surgeons interpretation is so critical. Blade: Many trans people say the world is too obsessed with whos had what done surgically. Do you agree? Bowers: Thats a crucial point and one that I keep bringing up proactively because obviously people still dont understand the difference between gender and genitalia. Gender, we know, gets established at a very early age, like by age 4, 5 or 6 and it doesnt really change very much. This is what transgender people have been saying for years, This is how I felt since I was 5 years old. So the question about surgery is really the dumbest question. I was a woman since I transitioned. Nobody tells you whether youre male or female. And it isnt about the surgery, its what society says when they meet you at the grocery store or the food counter. Blade: Trans acceptance seems to be making progress but still seems signicantly behind gay and lesbian acceptance. Do you agree with that? Do you think it will continue to improve? Bowers: Well, yes, I do think we are behind where the lesbian and gay community is in terms of acceptance. Some of that is just the sheer numbers, some of it is its still a little bit of a minority sort of thing and somehow it does sort of push peoples buttons in a different way. Thats too bad because if the gay and lesbian community saw the trans
community as more supportive, we could make much more progress but sometimes the discrimination we get within the gay and lesbian community is worse than it is with the straight community. Its like they just dont get it and its very hurtful. There are common threads that run through all kinds of discrimination. Were ghting the same forces that want to simplify the world and turn back the clock so everything is black and white and keep dragging at the heels of progress. Blade: What kinds of procedures do you do? All bottom stuff or more? Bowers: Kind of bottom plus. I do a procedure on the females, Chondrolaryngoplasty, which is a shaving of the thyroid cartilage. For some women, its a telltale sign in the throat and it was rst done by Dr. Biber in the 1970s. Its also a very delicate procedure thats not taught anywhere, no ear, nose or throat doctors do it. Its a very specialized thing. Blade: And you do both male-to-female and female-to-male gender reassignment procedures? Bowers: Yes. Blade: Which are more common? How many do you average in a year? Bowers: I do about 120 male-to-female surgeries a year. Its about four-to-one female to male versus male to female. Blade: Are most people able to orgasm after surgery? Bowers: Its different. For female to male, theres really no impact. With a Metoidioplasty, guys can use it for penetration so thats the good part there. If anything, its enhanced. Plus the fact that theyre testosterone-driven men, the libido tends to accelerate with transition. With male to female, its very complicated
and about 30 percent of biologically born women arent able to orgasm at all anyway. Our patients for the most part are able to. Its a very high percentile. About 90 percent but the thing you have to realize is that going from male to female for one thing, just hormonally, you tend to go to a lower level of interest just based on reduced testosterone levels. When youre a woman, you wonder why we leave men in charge of so much. Its so dominated by sexual thoughts. Sometimes I think, Wow, what was I thinking about all those years? Theres so much more to do. I say that sort of tongue in check. And the feelings are a big different. Maybe like going from the oboe to the banjo.
Trans Pride, now in its fth year, will be held Saturday at Metropolitan Community Church of Washington (474 Ridge Street, N.W.) from 10 a.m. to 5 p.m. and is designed to be an event where attendees stay for all or most of the day. Very early on, the community voted for it to be a health-based focus, says Danielle King, whos co-chairing this years event with Holly Goldmann. Thats why Dr. Bowers was invited. We felt like she could offer some insight that would be really attractive to our audience. A $10 donation is suggested but not required. King hopes attendees will plan to spend their day at the church. We want them to come and enjoy the whole day, she says. Therell be workshops and panel discussions on trans health throughout the day. She expects about 200 people, more than in past years when the event has been held at the Capital Pride host hotel. Trans Pride is under the Capital Pride events umbrella.

Taylor admitted that if the allegations against him had not been made he would not have come out. I would not be standing here today, he said, adding he considered ignoring the allegations. But now was the time to tell the truth, Taylor added. I felt like honestly this was it unless I spoke the truth, Taylor said, saying he didnt want someone to feel like they had something over his head. He said before today, he could count on one hand the people who knew he was gay. Taylor also said that he came out to his mother and family in the past 24 hours. He thanked his mother for her support and she hugged him warmly after he spoke to the media. I feel really good. My heart is as ease, he said about coming out publicly. Tonights sleep will probably be the best sleep Ive had in 12 or 13 years.
Taylor acknowledged that in the AfricanAmerican culture, being gay is a tough subject to discuss, which played a role in his not coming out publicly before. When asked about his political future, Taylor said, I am the same Rashad Taylor that was on the ballot in 2008 and 2010. My positions wont change. Taylor was anked by his mother and other family members as well as State Rep. Alisha Morgan; State Sen. Vincent Fort; Reese McCranie, the openly gay spokesperson for Atlanta Mayor Kasim Reed; Larry Pellegrini, the openly gay executive director of the Georgia Rural Urban Summit; and Georgia Equality Executive Director Jeff Graham. Taylor was greeted with applause when he walked to the podium. Taylor, who just turned 30 two months ago, is the youngest member of the Georgia General Assembly. He was rst elected in 2008 and then re-elected in
2010 to serve House District 55. Graham said he was pleased with Taylors announcement despite the circumstances. Im certainly very pleased for the community, and for Rashad. I think the circumstances are, of course, very difcult as he said, Graham said. I think anyone who identies as gay, lesbian, bisexual and transgender understands ... the one thing that ties us all together is coming out. First we have to come out to ourselves, then to family and friends and then eventually the public. That can be the most difcult process, coming out publicly, to strangers. He is embracing who he is. We have a community of hundreds of thousands of Georgians who will support him, Graham added. He has been a strong ally in the community. Im very proud we supported him in 2008 and 2010 and we gladly support him in 2012 as an openly gay man, Graham said.
Judge VAUGHN WALKER, who is gay, struck down Californias Proposition 8, but was criticized by conservatives for not disclosing his sexual orientation.

The Human Rights Campaign announced last week that it has ofcially thrown its support behind President Obama in his bid for a second term in the White House. In a statement, HRC President Joe Solmonese said his organization endorsed Obama because of what the president accomplished for the LGBT community during his nearly two-anda-half years in ofce. President Obama has improved the lives of LGBT Americans more than any president in history, said Solmonese. In 2008 we were promised change and profound change is what we got. More remains to be done and ensuring that President Obama is able to continue the forward momentum toward equality for another term is an absolute priority of the Human Rights Campaign. The achievements for the LGBT community that HRC highlighted in its endorsement statement include pressing for passage and signing legislation to repeal Dont Ask, Dont Tell; pressing for passage and signing a hate crimes law; determining that Section 3 of the Defense of Marriage Act is unconstitutional and refusing to defend the anti-gay law in court; and requiring hospitals across the country to grant visitation rights for same-sex couples. Alec Gerlach, spokesperson for the Democratic National Committee, said the HRC endorsement demonstrates the support that Obama has offered the LGBT community. That the Human Rights Campaign offered such an early endorsement is a clear sign that the president has fought for LGBT rights across the country and in our nations military, Gerlach said. We will work closely within the LGBT community in the months to come to ensure that we are united in the cause to re-elect the president and to ensure equality for gay and transgendered Americans. The president believes that DOMA is discriminatory and unfair, and

and have made throughout our history, will be fully recognized. Obama also noted that June marks the 30th anniversary of the HIV/AIDS crisis. The president recalled that last year he announced the rst comprehensive National HIV/AIDS Strategy for the United States, which focuses on using evidencebased approaches to decrease new HIV infections and to improve care for people living with the disease. This landmark anniversary is an opportunity for the LGBT community and allies to recommit to raising awareness about HIV/AIDS and continuing the ght against this deadly pandemic, Obama said. Obama has issued Pride proclamations for each of the three years in which hes served in the White House. Former President George W. Bush issued no such proclamation. But the proclamation fell short of announcing support for marriage equality. In a related development, White House Press Secretary Jay Carney was tightlipped on Tuesday when reporters pressed him for updates on President Obamas views on an executive order barring LGBT job bias and same-sex marriage. During a news conference, Carney said under questioning from the Washington Blade that he was unaware of any talks when asked if any internal discussion followed Sen. Tom Harkins (D-Iowa) recent endorsement of an executive order barring workplace discrimination against LGBT people. Not that Im aware of, Carney said. I dont have anything new for you on that. Earlier this month, Harkin came out in favor of an executive order that would mandate that the federal government only contract with companies that have policies protecting their LGBT employees from workplace discrimination. As chair of the Senate Health, Education, Labor & Pensions Committee, Harkin is the leading Senate Democrat on labor issues. Harkin joined Rep. Jared Polis (D-Colo.), a gay lawmaker and member of the House Committee on Education & the Workforce, and Jeff Merkley (D-Ore.), the sponsor of the Employment Non-Discrimination Act in the Senate, who have also expressed support for the measure. Carney said Obama welcomes Harkins input but has no information on the potential executive order. The Iowa senator helped build momentum for Obamas presidential bid in 2007 by inviting him to the 30th Annual Steak Fry, a major event for potential presidential candidates in the Iowa caucus process. Im sure that the president always wants to hear what Senator Harkin has to

say on a variety of issues, but I dont have anything for you on that particular one, Carney said. An executive order barring government contractors from discriminating against LGBT people is viewed as an interim alternative to ENDA passage while Republicans are in control of the House and progress on the measure in the lower chamber of Congress is unlikely. The White House hasnt said one way or the other whether Obama would be open to issuing such a directive.
Similarly, Carney said he had nothing to offer when another reporter asked why Obama has yet to come out in support of same-sex marriage. Mediaites Tommy Christopher said that a reasonable person would conclude, given the presidents recent statements and actions, that Obama in fact favors marriage equality although he has yet to articulate this belief. The president has said in the past what his position is, Carney replied. It hasnt changed. If it changes Im sure hell let you all know.
challenging the president rather than standing behind him. While Im sure HRC will claim they got lots of juicy promises in exchange for the endorsement, everyone else learned a long time ago that the president is unlikely to keep his promises unless you get in his face, and HRC will never get in his face, Aravosis said. So the promises are meaningless, and thus the president got HRCs endorsement for nothing, and now wont have to do anything for the next two years to truly earn that endorsement. Im sure it nails down the president for the next HRC dinner, but that really shouldnt be the goal here. HRC didnt immediately respond to a request for comment on whether the organization secured any additional promises from Obama in exchange for the endorsement. LGBT conservative groups also criticized HRC for making an endorsement before a Republican presidential nominee has been chosen or even before all the likely candidates on the Republican side have announced their intent to run for the White House. Jimmy LaSalvia, executive director of GOProud, said HRC is ending what he called its charade of bi-partisanship by endorsing Obama at this point in the election cycle. LGBT people who are interested in putting policy before partisanship now know that HRC is little more than a puppet of the Democratic National Committee and an organization that has one goal to elect more Democrats, LaSalvia said. R. Clarke Cooper, executive director of Log Cabin Republicans, also said HRC is offering its support too early by endorsing Obama with the 2012 election more than a year away. By prostrating themselves before Barack Obama eighteen months before the 2012 election, the Human Rights Campaign has effectively told the president that he doesnt have to do anything more to earn gay and lesbian votes, Cooper said. Given his lackluster record in the fight for Dont Ask, Dont Tell repeal, LGBT Americans were counting on HRC to hold the presidents feet to the fire on his other campaign promises, not to become a branch of his re-election campaign. Cooper further criticized HRC by saying the endorsement sends the wrong message to potential Republican presidential nominees who may want to reach out to the LGBT community. There are several possible candidates who deserve to be fairly
judged on their own merits, and the dialogue on equality issues for the 2012 campaign has barely begun, Cooper said. This decision makes it clear that Joe Solmoneses greatest priority is an invitation to drinks at a Democratic White House, not securing votes for ENDA, DOMA repeal or tax equity.
Such a pre-emptive endorsement is a mistake and will undermine equality efforts. In response to criticism from LGBT conservative groups, Sainz said HRC made the endorsement because Obama is far superior to any potential candidate the Republican Party may
choose in 2012. The records of other candidates seeking the presidency should be a wake-up call to all fair-minded Americans, Sainz said. As the ght for equality moves forward, President Obama is marching with us while the alternative would stop us in our tracks.

I asked what exactly are the charges [the counter-protesters were expected to be] getting. I was very interested to know what the charges would be. Nikolai [Alekseev] said it would be failure to obey, like what we get at the White House when we protest. Choi took this opportunity to get the counter-protester to think about the similarities between the two situations of being discriminated against merely for self-expression, which the former opponent began to seriously ponder. The processing took a while. There was a higher-ranking guy who came in and they seemed to be having a jurisdiction dispute and at one point there were more police in the room than any arrestees. And after a little more than an hour they took our passport numbers and released us without charges. The Russians were released later. Normally, as I understand it, with foreigners they get rid of you quick to avoid an international incident, but they hold Russians overnight and they get harangued and yelled at and abused. Instead they got a warning and were released. They knew exactly what this was about, they knew it was already getting attention in the news, and they wanted us to get out of there. The protest was at 12:30 p.m., we were released at 5 or 6 p.m. Everyone was
back by 9 p.m. Choi believes that the increased visibility being afforded the event each year signals a coming change in tides for the Moscow LGBT community. Six years theyve been doing Moscow Pride, the media environment has changed. People have been able to get things up on the Internet through Facebook, Twitter and things like that uncensored and as they happened. And, as you see in the pictures, there were more media people out there than protesters than both sides combined. Also this was happening at a time when [Russian President] Medvedev was giving a speech in front of the G8, and Tweeting himself that he wants more openness in the Internet and reporting, in speech, but at the same time, the country is banning freedom of expression for our community. In Russia even though its not illegal to be gay, they have a long way to go in terms of gay rights. They face the same issues as we do in our movement, but there are a huge number of people from all over Russia in Moscow who pack the clubs and a signicant part of that population remains closeted and even though in the city youll see that theres quite an acceptance of gays you wont see them holding hands and you wont see them protesting or agitating.

They acknowledge that so much has changed for the better over the past 30 years, including breakthroughs in biomedical research resulting in highly effective drugs that transformed AIDS from a death sentence into a manageable, chronic illness like diabetes. But AIDS activists also point out that HIV and AIDS continue to disproportionately impact gay men or men who have sex with men (MSM) in the United States and other countries. And although the perception of AIDS as a gay disease has largely receded from the minds of most Americans, AIDS activists say they nd themselves in the ironic position of having to remind Congress and state and local governments that more resources and funding are needed for HIV prevention programs targeting gay and bisexual men. MSM is the only group for whom, according to the CDC, new infections are still increasing, said Ronald Johnson, vice president for policy and advocacy for AIDS United, a national group formerly known as AIDS Action. So there continues to be a concern that there is not enough targeted prevention resources to MSM, particularly MSM of color and young MSM of all races and ethnicities, Johnson said. According to the CDC, while MSM account for about 2 percent of the U.S. population, more than half of all new HIV infections in the U.S. each year (53 percent) occur among MSM. CDC data also show that MSM make up nearly half of all people living with HIV in the U.S. 48 percent. CDC gures show that white MSM account for the largest number of annual new HIV infections of any group in the U.S., followed closely by black MSM, according to a CDC fact sheet released last month. There are more new HIV infections among young black MSM (aged 13-29) than among any other age and racial group of MSM, the fact sheet says. The Obama administration, with input from AIDS advocacy organizations, released a National HIV/AIDS Strategy document in July 2010 that, among other things, calls for an aggressive effort to develop better HIV prevention programs targeting MSM. Johnson and Carl Schmid, deputy executive director of the AIDS Institute, a national advocacy group, praised the administration for developing the strategy document, which they say covers most of the bases needed for addressing HIV prevention programs for MSM. But the two said the proposals in the strategy document have yet to be fully implemented. They note that delays in its implementation are due, in part, to the U.S. economic situation that has prevented needed increases in federal AIDS funds and severe cutbacks in state and local funding for AIDS-related programs.
Despite medical breakthroughs over the past 30 years, HIV infection rates remain stubbornly high among gay and bi men.

Phill Wilson, president and CEO of the Black AIDS Institute, said in a commentary last week in the Washington Informer, a black community newspaper, that he fears the horrors of the AIDS epidemic of the 1980s, when friends and family members watched loved ones die due to a lack of effective medical treatment, could return to some degree in the next few years. According to Wilson, if the federal government fails to boost funding for the federal-state AIDS Drug Assistance Program (ADAP), low income people who rely on the program to provide them the medications they need keep the AIDS virus in check could become casualties just as their predecessors became casualties years earlier. But this time, he said, an inability to gain access to medicine due to funding shortfalls would be responsible for their fate at a time when effective medicine is readily available. He called such an outcome immoral. The ADAP program was created under the Ryan White AIDS Care Act to provide life-sustaining drugs for low-income people with HIV and AIDS who are under insured or dont have any health insurance to help pay for the drugs. ADAP funding cuts by states and a large increase in the number of people applying for ADAP assistance has resulted in nearly 8,000 people being placed on state waiting lists for the AIDS drugs they need to remain healthy. The health insurance reform law that President Obama proposed and Congress passed two years ago was expected to relieve the ADAP funding pressure on states when it takes effect in 2014. However, some states that oppose the law have led lawsuits seeking to prevent its provision requiring all citizens to buy some form of health insurance from going into effect, making its outcome uncertain. Nearly all AIDS advocacy groups support the law, saying it would strengthen medical care for large numbers of people with HIV/AIDS. Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious
Diseases, an arm of the U.S. National Institutes of Health, has been the leading federal government ofcial monitoring the AIDS epidemic and directing AIDSrelated research since the disease burst on the scene in 1981. In a speech commemorating the 30th anniversary of AIDS at NIH headquarters in Rockville, Md., on Tuesday, Fauci said hes optimistic that an AIDS vaccine can be developed in the near future. We have scientic evidence that a safe and effective HIV vaccine is possible, he said in a statement released on May 18. In 2009, a clinical trial in Thailand involving 16,000 people demonstrated for the rst time that a vaccine could safely prevent HIV infection in a modest proportion of study participants, he said. Many of the best minds in HIV vaccine science are examining blood samples and data from the Thai trial to learn how the vaccine candidate prevented HIV infections and to consider how it could be modied to be more effective. Fauci said NIAID is also optimistic about development within the next few years of effective vaginal and rectal microbicides that can be used to prevent the transmission of HIV during sexual contact. Fauci and other researchers have also pointed to studies showing the effectiveness to a certain degree of prescribing HIV drugs for use by non-infected people believed to be at high risk for HIV infection, such as men who have sex with men. Known as pre-exposure prophylaxis, or PrEP, the use of this prevention measure is said to have the drawback of being less effective if people fail to take the drug as required. Some also have expressed concern that people using this prevention method are subject to potential side effects of the drugs and may be discouraged from using condoms, which experts say is one of the most effective methods of HIV prevention. Events and developments in the early years of AIDS 1981: The CDC reports in its June 1981 edition of MMWR and subsequent
editions that year that an estimated 170 gay men had succumbed to Pneumocystis carini pneumonia and Kaposis sarcoma, a rare skin cancer, over the preceding two years. The CDC studies of these cases cited a serious malfunctioning of the bodys immune system in those who contracted the conditions. 1982: Gay Related Immune Disorder, or GRID, became the rst name to describe what is now known as AIDS. Cases reached epidemic proportions in the U.S., moving beyond clusters of gay men in New York, San Francisco and Los Angeles and into groups with no obvious risk factors. 1983: Gay leaders, independent medical researchers and health and social services agency ofcials testify before a congressional committee that the federal response to AIDS was highly inadequate. They issue a plea for the federal government and the Reagan administration to increase federal funding and federal initiatives to ght AIDS. 1985: In late July, actor Rock Hudson stunned the nation when he issued a statement saying he had AIDS and was receiving treatment in Paris that he said he couldnt get in the U.S. He died three months later at age 59. His announcement and death drew massive mainstream media attention to AIDS. His death prompted his close friend, actress Elizabeth Taylor, to help found the American Foundation for AIDS Research to raise funds for AIDS causes. 1988: The NAMES Project AIDS Memorial Quilt makes its second trip to Washington in the spring, where its displayed on the Ellipse near the White House. Later that year, about 1,100 AIDS activists staged a protest at the headquarters of the U.S. Food and Drug Administration in suburban Maryland outside D.C., denouncing the FDA for taking too long to approve new drugs for people with AIDS. Police arrested at least 176 of the protesters after they blocked access to the FDA buildings main entrance.

I can tell you with 100 percent certainty, that many kids in D.C. schools do not get good health education.
because small children pull superheated liquids from microwaves. The doctor went on to say that one solution would help to reduce these accidents: families should turn their microwaves sideways. Since that time no lessons were created and disseminated to schools. No mail was sent to parents of students telling them how to protect their kids. We sometimes seem content to just to let kids burn. Which brings me to parents and teachers; they need more information and support. Talking to kids about sex is not easy. At MTA we often say that while we do sex education for a living, its still difcult for us. Yet only D.C. Public Schools currently has a clear and vibrant program to support health teachers in delivering sexual and reproductive health curriculum. And to make matters worse, the D.C. government does not certify health teachers. (Metro TeenAIDS has recently received funding from Venture Philanthropy Partners to expand our work in all D.C. Public Charter Schools.) Let me be honest about this. Despite my chosen profession, my husband and I have a 2-year-old and Im already nervous about the talk. Parents get little support when it comes to talking to kids about sex. And, of course, most of us did not have great role models. You wont be surprised to hear that things havent changed so much since you were a kid (no matter how old you are). But most parents want to do a better job. They want their kids to be safe. In a survey of D.C. parents we did in 2008, 90 percent of parents were concerned about the high rates of HIV and STDs and an equal 90 percent of parents said they were interested in getting training from professionals on how to talk to their kids. The research shows that when given training, parents who start conversations with their kids between 4th and 6th grade said they felt they had increased skills, comfort and condence to ensure open lines of communication with their children around sexual risk taking and that kids were more likely to wait longer before having sex. But what about once kids reach middle school and begin to put more stock in their friends than their parents? Well, for one thing, it is important for parents to know that even when your middle schooler gives you the look, they are still listening. But peers become more and more important as tweens become teens. Before he died, I had a chance to ask Dr. Martin Fishbein, one of the foremost researchers on behavior change, what he would do to end the HIV epidemic in D.C. He said the best return on investment would be to help change peer norms. But to do it effectively, wed have to do it at scale reaching into the social networks of youth and using marketing and messaging to inoculate youth against unhealthy peer norms. As an example, our own local data show that when youth start having sex they use condoms somewhat consistently but that over time, condom use decreases. And this trend continues into adulthood to the point that low condom use is one of the biggest drivers of our local epidemic. So what could we do to keep condom use high? While were just at the beginning of the effort, weve launched a new effort with the Global Business Council on HIV, Tuberculosis and Malaria to bring the likes of Nike, Axe body spray, Dove Soap and Gatorade to D.C.s table. Were not talking to them about an old style public health campaign. They dont sell product by talking about the benets of body spray. They make kids feel like they need body spray to get what they really want. And this is most likely where we can really make the most of the Internet and social media. I dont know how many more years of this epidemic well have. I do know that youth are getting infected early every day in our community. And I also know that unless we continue to push for change, ask for support and reach youth when and how they need us, we will continue to bury them. I miss Monica.

What mad Nijinsky wrote/About Diaghilev/Is true of the normal heart/... We must love one another or die, queer poet W.H. Auden wrote in his poem September 1, 1939. These words burned with a lacerating irony when playwright and gay activist Larry Kramers play The Normal Heart, whose title came from Audens poem, opened at the Public Theater in New York in 1985. Audens words retain their force today where The Normal Heart, which has received ve Tony nominations, is

experimentation using parts of the virus shell or another virus in order to trigger a protective response failed despite tens of billions of dollars in funding spent on it. As that door closed, another one has opened. We now know we can render a person who is HIV-positive virtually non-infectious. A widely reported recent study of couples in which one partner was positive and the other negative showed a 96 percent reduction in new infections of the negative partner when the HIVpositive partner was in treatment and on antiretroviral therapy a truly phenomenal result. So it turns out that we in essence have an AIDS vaccine. However, instead of administering the vaccine to the HIV-negative individual, we must give it to the person who is already infected. Treatment as treatment and treatment as prevention. Out of the billions of people who now populate our world, a fraction has HIV. If we nd them and treat them, new infections will plummet. This has now been conclusively proven. With or without effective prevention methods (which I wholeheartedly support) we can drastically reduce new infections among the negatives while also keeping tens of millions of HIV-positive individuals alive and well. Testing and treating is a winning strategy all around, and it is cost-effective to boot. Efcient methods of treating patients in poor countries for less than $200 a year are a reality. Each HIV-infected
person on the planet that needs medication and medical care could get it for the money that we are already spending on global AIDS. Here in the United States we need to nd the quarter of a million people who are HIV-positive and dont know it and get them into care and treatment. People who dropped out of care need to be persuaded to come back. Testing needs to be dramatically stepped up both in healthcare and community settings. Thirty years into this epidemic Americas response to AIDS is still a failure. From Reagan, who would not even mention the word AIDS for six years, to Obama who seems content to preside over an escalating gap in providing lifesaving medications to low-income Americans that now approaches 10,000 people, national leadership has always been sorely lacking. Today, the road ahead is clear: test and treat is how we can win the battle against AIDS. Marking thirty years of AIDS in America would be as good a time as any for President Obama and Congress to declare a true war on AIDS now that we know we have the tools to control it.

Community in Crisis
By DAVID HOFFMAN
On June 5, 1981, ve unusual cases of pneumonia in patients in Los Angeles were reported in the Morbidity and Mortality Weekly, a newsletter from the Centers for Disease Control and Prevention. It was the rst notice taken by federal public health ofcials of a deadly virus later dubbed HIV, for human immunodeciency virus that already claimed countless lives, but without a diagnosis pinpointing why. The plague had begun. It just didnt have a name yet. Now, 30 years later, the National Museum of American History, one of the Smithsonian museums on the Mall, the same museum featuring serious history and also pop culture, is marking this anniversary of what became known as the HIV and AIDS epidemic with a three-party display and website beginning June 3. With this rst ofcial notice of the illness, thousands were infected before anyone knew, says the exhibits curator, Katherine Ott, who has been working with an American History Museum team to plan it for the past year and a half. The full pattern simply didnt emerge until later. The public health and scientic as well as political responses to HIV/AIDS during the earliest phase, from 1981 to 1987, with original magazine covers and copies of reports plus lab equipment used to isolate the virus, are the focus of one of the three parts, the showcase to be in the museums Science in American Life exhibition area. Another set of materials, dating from 1985 through 2009 including movie posters, such as for the 1993 lm Philadelphia starring Tom Hanks and Denzel Washington will show in the museums Archives Center how individuals and society were affected by the epidemic. Finally, on the rst-oor Artifacts Wall, the museum will display a panel from the AIDS Memorial Quilt of the Names Project Foundation, honoring Roger Lyon, who died of complications from AIDS in 1984. A year earlier, Lyons testied before a Congressional hearing, throwing down a challenge to his listeners: I came here today to ask that this nation with all its resources and compassion not let my epitaph read that he died of red tape. But red tape and even worse, reluctance to face facts, often rooted in homophobia, led to denial that anything serious was happening and a crucial delay in addressing the epidemic. For years it was ignored by many in the political establishment, such as President Ronald Reagan, whose rst speech on the subject only came in 1987. Looking back at the early period, as the illness emerged and little was done to combat it, Ott, a lesbian, says I have friends who died, mostly in the 1980s, as a result. Theres a visceral aspect to this that other anniversaries just dont have, for people who lived through this, including those who became sero-positive in the 1980s and are still alive, says Ott, who also has friends and colleagues who are living with AIDS now. Anniversaries like this one are a perfect time to reect, to see where we are today, and how much things have changed, because its almost two generations now since the rst HIV reports. But in HIV years its probably more like seven generations. So much has changed. A month after the rst CDC newsletter notice came a short article in the New York Times, headlined Rare cancer seen in 41 homosexuals, and as the year 1981 ended, 121 deaths were recorded with the cause labeled gay-related immune deciency or GRID. The Grim Reaper indeed stalked gay men but as the scourge was fully understood, one caused by a virus passed through blood or bodily uid transmission, its dimensions were nally understood as constituting a global pandemic aficting both genders, and worst of all in sub-Saharan Africa. In 2009, an estimated 33 million people worldwide were living with HIV-AIDS, with 2.1 million fatalities from the illness that year and also with 2.6 million new infections every year. Today an estimated 600,000 men, women and children in the U.S. have lost their lives to the many complications from AIDS. Although numbers of those infected had been dropping in the U.S. earlier, that has begun to change in recent years. In 2009, District of Columbia HIV-AIDS ofcials reported that at least 3 percent of D.C. residents have HIV or AIDS. D.C. is now often called the nations HIV-AIDS capital. Ott, whose Ph.D. is in the history of science and medicine from Temple University, has been a curator at the museum for 15 years. She acknowledges that the exhibit has been extensively vetted within the museum, due to the controversy last year at a sister Smithsonian museum, the National Portrait Gallery, over the Hide/Seek exhibit there. She said Brent Glass, the History Museum director, because of sensitivity over that, had asked for a review of the HIVAIDS exhibit by the Castle, a reference to the Smithsonian headquarters. Glass now says that, this display will help visitors understand why these events gripped America 30 years ago. Ott says she hopes theres an audience for the new exhibit, especially for younger people whom she says have no idea really what we went through, and who now know that HIV infection is not an automatic death sentence, the way it was 25 or 30 years ago. She says she has interns today who have never even heard of Rock Hudson, the actor whose death from AIDS-related illness in 1985 made him one of the rst celebrities to die from the virus. She sees the exhibit as a chance for younger people to learn about how there were still sodomy laws in half of the states and how homophobia and the fear of this disease was so intense in the 1980s. Of course the virus is not specic to gay male sex, but because it was rst detected there, and spread so rapidly through the gay male community, it became linked in the publics mind to gay males as well as two other marginalized or outsider groups, Haitians and intravenous-drug users. The stigma was deeply set and led to laws being passed that discriminated against people with AIDS in insurance and housing and the workplace. These sanctions were linked to homophobia and often took the form of enforcing traditional attitudes about sexuality, as cities like San Francisco and New York took steps by 1984 to close down bathhouses and sex clubs. But gay males and their allies, often lesbians, began to act when others would not, to care for those stricken with the illness as well as to push back against vicious anti-gay ignorance with public education

HOW LONG HAVE YOU BEEN OUT AND WHO WAS THE HARDEST PERSON TO TELL? The hardest person I had to tell that I was gay was my good friend Jennifer but even though I didnt tell her until I moved away from South Carolina, the timing was what made us closer than ever. The hardest person to tell about my HIV status was my twin brother Kelvin and even till this day he has a hard time talking about it or your medication. He will come around eventually. WHOS YOUR LGBT HERO? My hero in the LGBT world isnt on a global or national level and really isnt a TV personality, its my great friend Kevin Nunley. He is always there for anyone at anytime. If it wasnt for him, I honestly dont believe I would be here today. And David Cantania who really needs no explanation, very awesome person! WHAT IS WASHINGTONS BEST NIGHTSPOT, PAST OR PRESENT? My favorite spot back in the day was Halo, but now Bear Happy Hour at Town, then Cobalt and little Number 9. DESCRIBE YOUR DREAM WEDDING. A barefoot wedding on Kiawah Island, S.C. WHAT NON-LGBT ISSUE ARE YOU MOST PASSIONATE ABOUT? Youth aging out of foster care and becoming homeless in D.C. I would love to develop a program that will link this group to employment and educational services beyond a GED (not that its a bad thing). WHAT HISTORICAL OUTCOME WOULD YOU CHANGE? I would change the outcome of Hurricane Katrina, if I could change that. I wouldnt want anyone to die or suffer in that way, not in the United States. WHATS BEEN THE MOST MEMORABLE POP CULTURE MOMENT OF YOUR LIFETIME? When A Tribe Called Quest broke up I thought I was going to die. I remember leaving class to get all their albums because we thought they were going to be instant classics. They are to me. ON WHAT DO YOU INSIST? That you keep an open and positive frame of thought in every situation, because you ultimately set the tone for your outcomes in life. WHAT WAS YOUR LAST FACEBOOK POST OR TWEET? Detox time until open bar at JR.s this Wednesday. IF YOUR LIFE WERE A BOOK, WHAT WOULD THE TITLE BE? Jack and Coke Please
CHANGE SEXUAL ORIENTATION, WHAT WOULD YOU DO? I would keep my orientation. WHAT DO YOU BELIEVE IN BEYOND THE PHYSICAL WORLD? I believe past this life, I will be able to live free without stigma and judgment. WHATS YOUR ADVICE FOR LGBT MOVEMENT LEADERS? Can we please stop with all the research facts and get to the human level of all of our problems? Scientic research isnt going to tell me my why someone really doesnt want to wear a condom or why they are afraid of getting their HIV test results. Lets see more community members on your panels and less of the weighty doctors. Although they do great work and continue to contribute a great deal, the guy in the waiting room tells a far different perspective of the epidemic and LGBT rights. WHAT WOULD YOU WALK ACROSS HOT COALS FOR? The chance to marry Gerard Butler, brunch at Masa 14 and the day that all LGBT people can walk into any church and exchange their vows before God, especially the churches I attended in South Carolina. WHAT LGBT STEREOTYPE ANNOYS YOU MOST? The LGBT stereotype that annoys the living hell out of me is how AfricanAmerican men are always portrayed in the media. We are either DL whatever that may be these days, or the loud HAAY GIRL and there is so much more to me than that. WHATS YOUR FAVORITE LGBT MOVIE? Noahs Arc Jumping the Broom (Noah and Carrie Bradshaw completely exemplify my love life) WHATS THE MOST OVERRATED SOCIAL CUSTOM? I cant think of one, but personally I hate all-white parties. Who the hell owns an all white outt and can you really eat the way you want if you have on all white? WHAT TROPHY OR PRIZE DO YOU MOST COVET? Bronze medal for my work at the EPA. And a teddy bear that my mom gave me in 10th grade after a horrible ght we had. WHAT DO YOU WISH YOUD KNOWN AT 18? To invest in real estate, Apple and know that the rst guy you date isnt going to be your husband. WHY WASHINGTON? Why not Washington with all the different cultures, races, backgrounds, etc., mixing and interacting on a daily basis? I have a great career, family that I have made and far more opportunities that I wouldnt have received elsewhere.

Stability balls can improve your core strength and balance and can even be used as an ofce chair.
A balanced workout
Tips for choosing and using stability balls
The easiest way to know what size to purchase is to test a ball. Sit with the small of your back on the ball with your feet spread hip width and at on the oor. Your hips should be level or just slightly higher than your knees. If you dont have access to a ball, use the following chart: 55 cm 411-54; 65 cm 55511; 75 cm 60-6 7. It is also important that your exercise ball be blown up to the right ination or poundage. Defer to the directions, which generally include a pump for the proper amount of air. Burst-resistant balls can be found at power-systems.com or balldynamics. com. Exercise balls are also now readily available at Wal-Mart and Target stores as well. The best way to thoroughly learn to use an exercise ball is through expert demonstration from a personal trainer. How to use an exercise ball: Exercise balls challenge you by putting your body in an unstable environment. When you sit or lie on a ball, you engage your core muscles such as your abs, lower back and hip exors to help stabilize while performing an activity. While balancing and engaging your core you can add numerous exercises to this element such as abdominal crunches and bench press exercises. By using the exercise ball as a bench you no longer have the stability of a standard bench, thus you additionally engage your core, butt and legs and you essentially get more bang for your buck. Whats more, there are progressions of difculty that you can implement such as balancing on one leg or doing one-arm exercises. And remember virtually any exercise you can do on a seat or lying on a bench you can do on an exercise ball. For abdominal training you can use the balls for just about any ab exercise you can do on the oor and then some. Exercise ball as furniture: Sitting on an exercise ball instead of a chair is an effective way for you to improve posture and constantly engage your core. Many companies that sell balls sell accessories such as wheels and back support, which as a progression can be taken away based on ability and core strength until you are just balancing on the ball.

Exercise fads come and go and there is always a new piece of equipment or tool that purports to do something unique and swift. It can be argued that such fads are mere ineffective attempts at a quick x and actually provide very little benet. This is not the case with the stability ball also know as a resist-a-ball or exercise ball. It was originally called the Swiss Ball and invented in 1963 in Italy by physical therapists. The exercise ball is by no means a fad and in fact has become a mainstay in most well-equipped gyms. Stability balls are also popping up in homes and ofces, and here I will give you the skinny on just what these oversized beach balls can be used for. The initial main uses for a stability ball were for abdominal and back strength, core stability and balance. Nowadays the stability ball has a host of uses including, but not limited to, using the ball as a bench that forces the core to work harder to stabilize the ball and the body while performing multiple exercises. The exercise ball can also be used for a variety of stretching exercises and for Yoga and Pilates movements. It is also used in more and more ofces as a chair that keeps the body in a uid state of motion and helps correct posture, not to mention the balance exercises you can perform while at work or on the computer as taking breaks from work is important. Here is what you need to know about choosing and using an exercise ball. Choosing the right size: Size matters.

Mistakes of memory
Tight Old Times production explores longing and frustration
By PATRICK FOLLIARD
Both Kates husband and her closest female friend want to possess her and her past. Obsessed with memories, the rivals have shaped recollections stressing their respective importance in Kates life. Whether their stories are even a little bit true is unclear, but in Harold Pinters enigmatic Old Times, now in production at Shakespeare Theatre Company, veracity takes a back seat to the power of relationships and the mistiness of memory. Seated in the starkly white living room of their quiet seaside home, Deeley (Steven Culp), a successful lmmaker, and his beautiful wife Kate (Tracy Lynn Middendorf) discuss the imminent visit of Anna (Holly Twyford), an old friend whom Kate hasnt seen in 20 years and her husband has never met. In fact, Deeley knows precious little about Anna, and the tidbits his wife has offered that Anna is not only her oldest but also her only friend; that they were once at mates in London; that she used to steal Kates underpants have done nothing to endear her to him. Anna arrives. Over drinks (and cigarettes), she and Deeley genteelly set to battle while Kate curled up on the couch like a cat looks on rather amusedly. Initially Anna witty and sociable recounts the fun times she and Kate shared as single young women in London. In reply, Deeley relays how he rst met Kate at a grungy cinema where theyd both gone to see the aptly titled Odd Man Out. Anna remembers things differently. She says she saw the lm with Kate but Deeley wasnt there. Eventually, the conversation escalates into a heated contest of who knows Kate best. They even compete in singing the half-forgotten lyrics of Kates favorite old songs. When Kate leaves the room to bathe, Deeley says he remembers Anna from their London days. He claims they drank at the same pub, and one night he even peeked up her skirt at a party. Initially Anna says it cant be true, but then changes her tune, admitting she was at the party wearing Kates underwear. What to believe? At one point Anna says, There are some things one remembers even though they may never have happened. While Deeleys stories about Kate are tinged with sex (he tends to objectify her), Annas intimate moments/memories are more gentle she strokes Kates arm while asking if she might draw her bath or read aloud to her. Anna now lives as expat with her husband in Sicily, or so she says. Here, very little is for certain. But clearly she was at one time or still is in love with Kate and Kate seems to respond positively to her expressions of tenderness. Feeling relegated to third wheel status, Deeley grows increasingly angry and is ultimately reduced to heaving sobs. Impeccably staged by Shakepeares gay artistic director Michael Kahn, the production is taut and very funny. The three-person cast is rst rate. Making her debut at Shakespeare, Twyford (who is also gay) handles the brief, precise Pinter dialogue beautifully, and rather hilariously lls the playwrights well-known pauses with expectant frozen smiles. Jane Greenwoods costumes nicely illustrate Anna and Kates differences: In addition to a slick chignon, the former wears a rather severe dress with a bow at her throat while the latters casual peach clothes and loose-tting pink robe describe a softer, less dramatic person. Deeley is appropriately tweedy as the accomplished documentarian relaxing away from town. Walt Spanglers bright white set makes a perfect blank slate on which to scribble and erase memories, real and imagined.

You tried everything. When you had trouble with insomnia the other night, it seemed like nothing would lull you back to sleep. Warm milk tasted bad. That previously boring book on your bedside table suddenly turned intriguing. Even infomercials held your interest, so you started counting sheep. Then you got to wondering why sheep? Why not count cows or dogs? Is it because sheep are, well, like sheep? Author Catherine Friend wondered that herself because she has a ock of them on her Minnesota farm, and in her new book Sheepish, she writes of the good and the ba-a-a-ad, the wild and the wooly. Though her grandmother raised them on a Montana ranch, Catherine Friend had little experience with sheep that is, until her partner, Melissa, wistfully admitted her dream of owning a farm and raising the critters. And thus it came to pass that Catherine had a little lamb. Fifteen years later, Friend has morphed from City Girl to Backup Farmer. It hasnt been a gentle-as-a-lamb transformation, but Friend now appreciates her ock. Ovines have a long history in North America, she says. Sheep were shipped to the New World in 1609 and within 60 years, there were more than 100,000 sheep on our shores. English lawmakers tried

campaigns to promote what came to be known as safe sex. The museum exhibit showcases this entire period, with a focus on several of the stages rst, as a public health crisis, when the word was being passed about how AIDS was transmitted and how to prevent
the diseases spread; and then as a scientic mystery, when a high-stakes international race began, to nd the cause of the illness; and also as a political ashpoint, when some voices were raised condemning homosexuality as a sin and sometimes even that AIDS was a suitable punishment for such infractions. Other voices challenged Reagan administration inaction, especially in
1987 when activists took to the streets in civil disobedience through ACT UP the AIDS Coalition To Unleash Power, with its familiar pink triangle and its emblematic slogan of Silence = Death. Depicting these stages are magazine covers such as one from The Advocate in 1981, with its front-page headline question, Is The Urban Gay Male Lifestyle

Hazardous to your Health? about the new diseases attacking gay men. Another cover story headlined Epidemic, about the mysterious and deadly disease called AIDS appeared in Newsweek in April 1983. Ott says that its lead author decided to focus on the illness partly as a result of the fact that he had a brother who was gay and who later died from AIDS complications. Other items on display are the early reports and pamphlets charting the course of efforts to combat the disease. The museum has collected what Ott calls a lot of the paper ephemera, much of which was educational, to get the word out, including efforts to promote safe-sex practices, such as a booklet titled How To Have Sex in An Epidemic, co-authored by Richard Berkowitz and Michael Callen, advocating the use of condoms and other measures for safe sex; and a pamphlet from 1982 titled Healthy Sex is Great Sex. Also shown is an early safe-sex teaching manual, Teaching AIDS: A Resource Guide on AIDS, co-authored by a social worker Marcia Quackenbush, whom Ott says began working in sex education with gay youth in San Francisco and shes still active. Quackenbush in fact will be among the bloggers at the exhibits website, which will log in new comments from experts about the exhibit once or twice a month. The exhibit is expected to run until November, though no denite closing has been set yet; it will depend on when the museums West Wing, where the materials on display will be housed, must be closed for renovations. Some of the displayed material comes from the extensive collection of research materials donated in 2008 to the museum by the writer John-Manuel Andriote, author of the 1999 book Victory Deferred: How AIDS Changed Gay Life in America. Andriote, who lived in Washington for several decades until returning home to Connecticut in 2007, completed in 2010 a series of interviews to update the book, which will be re-issued later this year in a new edition. Andriote, who learned he was HIVpositive in 2005, says that in his interviews last year he was, Startled to hear gay men now say that their sense of the crisis has passed, that HIV has become normal, a harsh fact of life but not what denes or necessarily ends it. The exhibit also displays antibody tests from 1985 and condoms distributed to gay men in the early and mid-1980s, with a stress on the importance of their use to combat AIDS. One section shows how those test kits emerged from the research in 1983-1984 of two physicians, Luc Montagnier (Pasteur Institute, Paris) and Robert Gallo (National Institute of Health, Bethesda), whose pioneering lab work led to them being recognized as the codiscoverers of HIV as the virus responsible for the cause of AIDS. They co-patented the test kit and split its royalties.

sell the loan to another investor or on the secondary loan market. You dont have to make a loan for the entire purchase. Remember the 8010-10 loan where the buyer contributed a 10 percent downpayment and a bank or mortgage company funded 80 percent? You can offer to finance only the remaining 10 percent or some other portion of the total sales price based on your comfort level with the buyers credit and payment history. You may quickly dispose of an otherwise unsellable property that does not qualify for traditional financing in line with Fannie Mae/Freddie Mac or private mortgage insurance underwriting guidelines. A buyer may offer a higher price for your home just for the privilege of obtaining private financing. Even if you find that the risks of sell-
er financing outweigh the advantages, check your loan documents to see if your loan is assumable. Mine is. This is also common for many FHA mortgages and allows a buyer to take advantage of a streamlined process to ante up the difference between the sales price and your current loan balance and take over your remaining payments, potentially saving hundreds or even thousands of dollars in closing costs and interest. As credit restrictions grow in the wake of short sales and foreclosures across the country, well-qualified home buyers continue to need more options. Consider being the banker that blue, pin-striped suit might look good on you.
