Behavior Contract

I, _______________________________________ , agree to follow the expectations on the

behavior checklist to the best of my ability. I understand that if I follow the
expectations, I will be rewarded with computer time. If I do not follow the
expectations, I will not be able to get computer time. Over time, my teacher will
increase the amount of time that I have to follow these expectations in order to have
computer time.

Behavior Checklist
At first, Justin will have to follow the behavior checklist from morning until
lunch. He will have to follow 3 out of 4 behaviors on the checklist to get a sticker,
which means reward of 15 minutes computer time. The same will apply for lunch
until the end of the afternoon. Over time, Justin will have to follow all 4 behaviors on
the checklist in the morning to receive a sticker, and therefore computer time. He
will have to do the same in the afternoon to receive a sticker and computer time
again. Eventually, the goal is that Justin is able to follow these behaviors for the
entire day before earning computer time at the end of the day.

Stay in my assigned seat for at least 10 minutes before getting up.

Work on/complete tasks to the best of my ability during independent or
group work.
Remain focused and quiet during independent work.
Follow directions given by teacher.
