5 STAR FORM

Point Based System:

Each member is required to maintain a record sheet of any points accrued and have the
advisor or an officer sign off during the activity or event. You must also sign in at each
event or activity to be credited for the points, whenever there is a sign in sheet
available. In order to achieve your star level you must accumulate the following amount
of points throughout your time as an active member in the Alpha Mu Tau Chapter.

STAR LEVELS
One Star Level 20 points Gold Tassel
Two Star Level 40 points Graduation Honors Stole
Three Star Level 60 points 20 Honor Dollars
Four Star Level 80 points 30 Honor Dollars
Five Star Level 100 points 50 Honor Dollars

Honor Dollars are to be used anywhere on campus (Book-Store, Café, etc.) or towards
academic purposes. They are issued as a Phi Theta Kappa VISA Gift Card.
Each time a star level is achieved, the reward listed for each level will be given.

Point System:

Attend New Member Orientation 5
Attend Induction Ceremony 5
Attend General Member Meeting 5
Attend an Event the Chapter holds 10
Volunteering to help at an Event 10
Chair or Co-Chair a Committee of the Chapter 15
Participate on Committee (Each Committee Meeting Attended) 5
Take SLA Course 50
Attend a Regional/District Meeting or Event 10
Run for Officer Position (during Spring) 10
Providing a helpful suggestion for the Chapter 5
Presenting an Event Idea for the Chapter 5
Having correct answer to weekly Facebook Trivia Question 5
Bring a Member to an Event/Meeting who’s never been before (minimum of 5) 5
Introduce an eligible student and get them to become a member. 10

Letters of recommendation will only be given to members with a 3 star status or higher

5 STAR FORM - Sign Off Sheet
Maintain this sheet for your records. You must complete the event information (Event,
Points, & Date) before you request a signature from a chapter officer. Incomplete event
information will not be signed. Remember, it is your responsibility to maintain and keep
track of your record of points.
