This is a follow-up of my letter on November 18, 2010 (copy hereto

attached) for release of amount to me as a subscriber of College
Assurance Plan with assigned number 0019635 and SFA Account No. 60-
71-18-01-00124.

I was informed that I have to make a statement that I am

surrendering my CAP Policy as a pre-condition for the release of the
amount due to me.

In this connection, I hereby state that I am surrendering my CAP

Policy which I earlier submitted.
