MINUTES Site Organizational Chart and Chain of Command. MKD to submit organization chart by Wednesday 5th December 2012 complete with curriculum vitaes. For vacant positions MKD to advise on expected date to be filled. MKD yet to complete organization chart as MI note some positions are still un-filled. Urgent positions for structural engineer and RC schedulers needed to be filled immediately as MKD must start to produce shop drawings prior to commencing the permanent works. Note: MI instructs MKD that all site personnel must be present on site and responsible for duties. MKD confirms that organization chart 90% completed. MI highlighted on urgent position for scheduler must be filled as soon as possible. MKD confirms that still on process to fill up this position. One person base on MKD main office with MKD project director will responsible for this duty until position filled. Note: Done. MKD confirms this position already filled. Method of Statement.

MINUTES MKD to submit method statement for : Breaking piles/Excavation/List of proposed main structure by 7th December 2012. Note: Done. Site settlement/ Formworks/Concrete works/Surveying by 12th December 2012. Note: Done, review by MI. MKD to provide list of method statement with date of submitted and approved. All submission must be submitted to MI head office. Program. Overall program will be submitted on 12th December 2012. For Bran pollard warehouse MI instruct MKD to spilt schedule between 4000m2 and the rest of warehouse building. Note: Done, MKD submitted main program and flour mills building program base on meeting which held on Friday, 4th January 2013 with BS. MI requested for special meeting to discuss about this program as MI has comment on the program, Wednesday 16th January 2013. MKD to start producing S Curve and construction progress together with weekly report. Related to steel frame work for bran pollard (refer to coordination meeting Blue Scope/Gerbang/MKD/PBS/MI on 18th Dec 2012): 1. 4000 m2 (Gl. A I) : - Breaking pile should be finished 2nd Jan 2013(Done). - Pile cap and tie beam should be finished 16th Jan 2013(Still on progress). 2. 8000 m2 (Gl. J W) : - Breaking pile should be finished 9th Jan 2013 (Still on progress). - Pile cap and tie beam should be finished 6th Feb 2013(Still on progress). Urgent matter : 1. Related to temporary drainage sloping pavement by river stone west side should be finished 31 Dec 2012 (Done). 2. Mock up for permanent fence should be finished on 31 Dec 2012 (Done). 3. Site Office should be finished on 13rd January 2013(Still on progress). Delayed to 19th January 4. Hold area may continue using embankment system and reduce number of motorcycle parking lot. Not confirm with final drawing form MI 5. MKD to continue with fence construction. MKD informs that Lion mesh production will be delay for 1 month. Scheduled about 30% of material can delivered by 21st this month 6. BS confirms sliding gate will be use for both, front and back entrance. 7. BS confirms traffic study given by BITA. MKD to submit weekly works planning completed with percentage completed for each items.

MINUTES Material Approvals. Schedule of material approvals to be submitted partially by next week. Note: Outstanding from MOM 005. Shop Drawings. List of shop drawings to be provided highlighting when this will be submitted in line with construction program. Note: Done for submission until 6th Jan 2013, MKD to be submit progressively for the other. MKD to submit set up site grids and transfer information on to site dimension. Note: Done for Bran Pollard Warehouse and Flour Mill Building, on progress for others. Equipment/Machinery/Batching Plant. Batching plant on site will start commencing by second week of January (tentative date). MKD informs that they will use batching plant from Siam Cement Group (SCG). MKD need to clarify resources for raw material with SCG in order to support batching plant and meet MI standard quality. Meeting to be held with SCG tomorrow, MKD to advise all on outcome. Note: SGC started on 8th January 2013 and should be finished by 28th January 2013 still on progress MI reminds MKD a full technical submission will be required for tower crane. Note: MKD has already issued tower crane design calculation, MI has reviewed and comment. MKD to follow up and update. Still on progress ordered material piles Manpower. MKD to submit list of worker and sub-contractors that involved with site activity. Separate meeting will be held to discuss about Man power on Friday, 7th December 2012 at MI office. Note: MKD confirms to submit by 5th January 2013. Outstanding from MOM 005. Health, Safety and Environmental MKD to ensure construction activity comply with health, safety and environmental regulation. Note: MKD reminds to be strict on safety issue as MI notes that breaking down pile didnt comply with safety regulation.

MI reminds MKD the aim of this is to keep your team focused and in order to get the correct procedure/method of works/documentation to facilitate permit to work to be issued successfully by the MI CM team. Note: Outstanding from MOM 005. MKD to review and respond minutes of meeting(MOM) latest 24 hours before next meeting. MKD to conduct daily meeting and day to day coordination/priority issues with MI CM. Minutes of daily coordination meeting need to issue as reference.
