K/1 Fieldtrip: Tallahassee Museum Permission

Where: Tallahassee Museum: 3945 Museum Dr.
When: Friday, November 18th at 11:00. The students leave SASC by bus at 10:30 am and return
around 1:30 pm.
Cost: $10 total ($10 per student ticket, PTA is covering the bus! Thanks PTA)
My child _________________________has permission to ride the bus and participate in the tour of the Museum
on Friday, November, 18th and I have enclosed $10 to cover the cost.

Chaperones: All parents are welcome to join in for the tour of the museum and join us for lunch but only
requested chaperones will be asked to join the tour of the Farm due to limited space. Chaperones are
required to provide their own transportation to and from the Museum as well as their general admission at
arrival.

lunch. I understand that I will pay general admission to the Tallahassee Museum on arrival.
