Self-Assessment

I chose to complete a healthy lifestyle assessment that determined if I was at risk for noncommunicable diseases (NCD). NCDs, also known as chronic diseases, are not passed from
person to person. They are of long duration and generally slow progression. The 4 main types of
non-communicable diseases are cardiovascular diseases (like heart attacks and stroke), cancers,
chronic respiratory diseases (such as chronic obstructive pulmonary disease and asthma) and
diabetes (Unknown, 2015). After completing the assessment, it was determined that I was at a
medium risk for having an NCD due to some unhealthy behaviors. Based off of this assessment,
I can make some changes by adding more fruit and vegetable servings to my diet, and add more
physical activity to my daily routine. These strategies for change can help provide me with a
healthier future and lasting mental and physical state. The assessment questions were as follows:
1. Do you eat 5 or more servings of fruit and vegetables daily?
2. Do you get 30 or more minutes of exercise a day?
3. Do you drink more than 1 alcoholic drink a day?
4. Have you ever used tobacco?
Although I am considered to be at medium risk for an NCD, I feel that I have strengths as
well. I am aware that I need to live a healthier life, and in the past 6 months I have started seeing
a nutritionist to help with meal planning and exercise routine. Due to my current actions, I feel as
though the assessment scores are true, however I am slowly working my way to a more healthy
lifestyle. I can use this information to lead a community towards a more healthy and active
lifestyle by sharing my results and feelings, as well as creating community physical activity
classes such as Zumba.
