Transcript of Records (TOR) on my behalf. I am currently on work assignment at the Bureau of
Customs, Port of Cagayan de Oro, Cagayan de Oro City and I would need the original copy of
the above named document to complete the enrolment process at the MSU-IIT College of Law,
Iligan City. Classes will start on August 03, 2015.

Attached with this letter are the photocopies of my company ID and my drivers license.
