I, the undersigned, MA. ANGELICA C. OBREGON, Filipino, legal age, presently residing in
Brgy. Canlapwas, Catbalogan City, Samar, herby authorized my sister MA. MARIEL MAY C.
OBREGON, who is a professional license holder herself, to facilitate in my behalf the
authentication of my Professional License Card.

The authorization given to MA. MARIEL C. OBREGON is due to travel restrictions imposed
by the City of Catbalogan through AITF guidelines. This is in line with Executive Order No. 09-
002, s. 2020, “Promulgating localized guidelines on the Application of Zoning Containment
Strategy in Catbalogan City”, issued on September 23, 2020 due to the continued increased of
COVID-19 infection.

Moreover, I have no objection for her signing documents if there is any. I will enclose copy of my
identification to prove that this request is valid. Let it be known that she will present valid scanned
documents which I sent via email.

For security purpose, she will hereby present and/or provide copy of his professional ID if deemed
