SUBJECT : Request for Reassignment of PSI JULIUS CARLO B BELMES

DATE : January 16, 2018

1. The undersigned humbly request the reassignment of PSI JULIUS CARLO B

BELMES to this station where he is projected to be designated as Chief, Intel/Tracker
Tem Leader on Focused Team for our campaign against wanted persons and criminal
groups/ elements and will be designated as Chief, SWAT/EOD/K-9 Unit upon activation
of said unit at this station.

2. That his assignment to this station will greatly help in solving our problems
against criminal groups/ elements who are suspects of murder and shooting incidents
as well as improve our capability to counter terrorist activities because of his available
K-9 and possible conduct of K-9 Units Training in our AOR with the support of the City
Government .

3. Request for favorable action.
