The purpose of this evaluation is to communicate clearly to the individual evaluated how well he /she is
meeting expectations for a person at his/her level. Comments should be specific (including examples)
and explanatory.
Rating Scale: A Five-point rating scale is used for the purpose of assigning a final evaluation score to the employee. This
is described below:

Far Exceeded Expectations (FEE)
90%-100% Accomplishments are rarely equaled, are clearly obvious to management team and key work partners and
serves as a role model; indicating strong potential for rapid future development
Exceeded Expectations (EE)
76%-89% Consistently achieves high levels of commendable performance and is recognized as highly effective by
management team and key work partners; indicating potential for future advancement
Met Expectations (ME)
60%-75% Overall, a solid performer who is recognized as effective by management team and key work partners.
Performance may exceed expectations at times
Partially Met Expectations (PME)
45%-59% Minimum job requirements are accomplished; however, some progress is required in order to consistently
meet performance expectations
Did Not Meet Expectations (DNME)
31%-44%
31%-44% Performance is below acceptable level and immediate improvement is needed in order to reach an acceptable
level of performance.
