Direction: Please read the attached Questionnaire-Checklist including all the directions
and items. Then read each indicator statement in the Evaluation Sheet and rate each item
by making a check mark () on the appropriate column, using the rating scale below.
RATING DESCRIPTION
A Highly Valid (HV) No flaws observed; nothing more to be desired to
make it better
B Valid (V) Very little flaws observed; minor rewording of the item
needed
C Moderately Valid (MV) Some flaws are observed; the overall usefulness is
diminished only slightly
D Slightly Valid (SV) Several flaws are observed; overall usefulness is
greatly diminished
E Not Valid (NV) Major revision is necessary to make it useful

2. Each item is clearly stated.
3. Each of the items is readable i.e. item is easily read.
4. Each of the items is attractive to read; enough space is

covered all areas that are important in the study.
6. Each item is focused on one particular thought or idea.
7. The items are objective, i.e. the responses to be elicited are
neither biased nor reactive.
8. The items are formulated in accordance with the
explicit/implicit objectives of the study.
9. The items are systematically arranged according to a
desirable sequence.
10. The items do not overlap with each other; no duplication is
observed.
