SERVICE REQUEST APPROVAL NOTE

DEPARTMENT: Electrical Maintenance

SUBJECT: ARC of maintenance, rewinding & repairing of Plant open well & mud submersible pump
motor assembly.

CONTRACT TYPE – AMC/ONE TIME/SERVICE/RENEWAL: AMC RENEWAL/ARC

COMPLETION PERIOD: From 01.04.2018 to 31.03.2019

The Documents shall be accompanied with the request:

1. Scope of Work shall be mention clearly along with service request. (Both DIL & Vendor)
2. Separate BOQ & Price shall be required in case of Supply and services involved for any type of job.
3. Initial offer & Final offer shall be required in case of negotiations. (Mail & offer- communication)
4. Minimum 3 Offers required from the different service provider to perform the job
5. Any New vendors introduce for Service or Supply should have proper credentials or at least having similar type of work experience.
6. Communication mail should be attached with every note. (DIL & Vendors communication)
7. Any single offer required for expert services or any offer obtain to comply the force majeure requirement, same clarification need to
be mention in note.
 Scope of work:

1. Vendor shall deploy trained and experienced manpower for carrying out the maintenance
job of submersible and dewatering motor pump assemblies.
2. After completion of maintenance job, all required testing and no load trail run of motor is
in agency scope.
3. After maintenance of Submersible proper tagging and painting of motor is in agency
scope.
4. Vendor shall provide the six month guarantee in repaired motor pump assembly. If motor-
pump assembly will fail within guarantee period the vendor shall repair the motor free of
cost.
5. All required T&P for carrying out maintenance job is in Agency scope.
6. Transportation of manpower and Dewatering pump assemblies for maintenance purpose is
in Agency scope.
7. The payment shall be made as per the actual work as certified by Officer-in-charge.
8. Transportation of material, lodging, boarding, food and travelling shall be in agency
scope.
9. Workers compensation policy shall be submitted to M/s. DIL on commencing of ARC.
10. Agency shall be responsible for any incident (if happened) at site during the work due to
non-compliance of safety from their side.
11. Agency shall ensure all safety precautions while working on equipments.
