at Zone 7 Lower Bulua, Cagayan De Oro City, Philippines do hereby APPOINT SIEGFRIED H.
TAVEROS Attorney-In-Fact, single, likewise of legal age, with postal address at Guadalupe,
Cebu City, Philippines as my true and legal representative to act for and in my name and
stead and to perform the following acts:

To act in my behalf for my request/application for my original University of

Cebu- Graduate School Official Transcript of Records and two (2) copies of
Certified True Copy of the same document duly Certified, Authenticated and
Validated (CAV) by the Commission on Higher Education (CHED).

HEREBY GRANTING unto my representative full power and authority to execute and
perform every act necessary to render effective the power to request, apply, sign
necessary forms and claim the said documents, as though I myself, have so performed it,
and HEREBY APPROVING ALL that he may do by virtue hereof with full right of
substitution of his person and revocation of this instrument.
