With Reference to DAR Engineering letter Ref # ASH-DAR-PBC-GEN-LTR-869 Listed below

are the actions taken to rectify those problems noticed by DAR Engineering at site.
1. ASH- DAR –PBC- HSE –LTR-855 – Safety & housekeeping (HSE-NCR-131) on 02/01/2019 is
in progress as debris and wastes are removed on daily basis, waste materials staked will be
allotted for waste material storage area.
2. DAR Safety Corrective Action Request –ASH-SCAR-0064 on 19/01/2019 had been
completed –See attached copy of photos.
3. DAR Safety Corrective Action Request –ASH-SCAR-0066 on 24/01/2019 had been
completed – see attached copy of photos.
4. DAR Safety Corrective Action Request –ASH-SCAR-0067 on 29/01/2019 had been
completed Except TUV for Man Lift operator, This TUV will be renewed in due course once
financial problem of PBC Company is solved.
5. DAR Safety Corrective Action Request –ASH-SCAR-0068 on 04/02/2019 had been
completed –See attached copy of photos.
6. ASH- DOR – HSE 002 – Daily HSE Observation Report – 19/01/2019 had been completed -
See attached copy of photos.
7. ASH- DOR – HSE 002 – Daily HSE Observation Report – 20/01/2019 had been completed -
See attached copy of photos.
8. HSE Weekly Walk Through findings -29/01/2019 is in progress

With Reference to ASHP and DAR HSE NCR # ASH – DAR-NCR-HSE –

NCR-131, Housekeeping work is in progress and is continued on daily basis.
Unwanted materials are removed from each floors and those wanted materials
are properly arranged and stocked in order to prevent slip and trip hazards.
Combustible materials like which are stacked are provided with fire
extinguishers to prevent fire hazards. We also provided daily housekeeping
program which is in progress.
