Things to Do/Follow-Up

 Rig move is ahead, road risk assessment done shared with office, and pre-rig move meeting is pending
which will be done once movers are on location.
 Rig Audit by OMV Vienna is expected. Pre-hire rig audit checklist. (Updated Copy on the board).
Company man will finalize the checklist but HSE has to facilitate him.
 Mast beams TDS and draw works motors will be changed on rig move.
 Cat 3 inspection of mast is planned during the move.
 Rescue Drill Procedure from Monkey Board received and conducted. We will do it on next well spud.
 We have started some extra gifts of appreciation for the people who are writing regularly, for that Franz
arranged BP wrist apparatus and some of the dairies, last time we choose card from catering, time we
have good number of cards from security guards. In one of the draw we have 4 BP apparatus are left
which we plan one for every month.
 Well end report I started but most of the data will be load after rig release. The day rig will be released we
will make reports of the Latif-14.
 MPI of TDS is expired, new TDS arrived on location but Saxon has to provide the scan copy of each
certificate of the new TDS.
 We will be having ISO 14001 Audit in March.  Internal environmental Audit conducted on 26th December
2015 Report submitted to Adnan & copy is on desktop EMS folder.
 Best Strive cards, management meeting and management walk around for January are done.
 Best Strive card will be given with the recommendation of company man.
 Web slings were cut and disposed off as per OMV standard, we will stick to same standard.

Inspections:
 Geronimo & line tested by 3rd party, report submitted to Adnan bahi
 Lifting gear register updated and fall arrestor certificates are provided by Saxon ( mail box)

ATR:
 ATR need to update and close out open points on target date. Owner of the ATR is DSV but we have to
update and send to company man for his approval.
 Before closing ATR points please discuss with company man.
 We will keep send ATR every Sunday as we were doing before.

ERP for Camp:

 We had small gas leakage last day ERP was followed details of Sawan teams are in mail box. Latif-10 is
flowing well and camp on Latif 10.
 Plant will initiate ERP for well, we will follow our ERP plan.
 In case of any issue with production installation, well operator is visiting on site every day.


Reorganizing HSEQ Document System:

 The main document is name Latif-5ST and I have placed it on desktop, all updated documentation is in
this folder. If something is missing please check in Minano-19 folder if you don’t find Miano-19 folder call
me.

Meetings:
 Daily morning meetings are 8:45 AM with company man.
 Daily Conference Call 9:00 AM
 Weekly meeting done

Reports:
Daily QHSE Report:

 We have to submit the report before calling off the day to co man, after adding information from SLDC
QHSE report which they will email to you about 05:45hrs and POB will be sending by Rig Doctor. Made
few changes as per drilling manager instruction we have to add stop cards details in report which we send
to everyone in the list.
(Please Send POB to Adnan on daily basis) No one is allowed to stay on site more than 35 days
maximum, only catering crew can stay up to 45 days, Security 60 days.

Man-hour calculator and vehicle record is in HSE reporting folder, kindly use it to have proper man hours
and vehicle record.

Drills conducted this week

For drill we have to follow New KPIs of 2016. Details of KPIs are sent by Adnan
 Fire drill @ rig site. Done
 Fire drill @ camp site. Done
 H2S drill @ rig site. Done
 Stretcher drill@ rig site. Done from monkey board is also done.
 Spill drill@ rig site. Done
 Kick drill@ rig site. Done
 We should have designated Radio set for emergency, already in discussion with Adnan bhai, please
follow up.

WEEKLY QHSE Report:

We will send contractor presentation of every week as we were doing before.
Send weekly HSSE report and a presentation which is a contractor meeting presentation. Contractor meeting
presentation (All the data and tables are in same folder of contactor meeting presentation) we have to send by
Sunday noon cut off time for presentation is Saturday of every week. Rest ATR and weekly report will be sending
on Monday as before. (For reference check e-mails)
These training hours you will get from SDS QHSE & Doctor, include drills (Fire or H2S) and Rutledge H2S
induction.

Monthly QHSE Report:

It is a simple report just data entries, like; Monthly report will be submitted to distribution list below in Well End
Presentation;
CARE input numbers (Client)
Training Hours (Client & Contractor)
Vehicle KM (Client)

Service companies 3rd party certification

Medical certificates are must who so ever is coming on location, we didn’t allow anyone without medical
certificates, requirement to enter on OMV facility are drafted by Albert and Adnan bhai we have to follow those.

future in case of any incident.

H2S Equipment Details

 Rigged up & fully operational,

 Refresher training is ongoing with site induction as well.
 We have to send all the equipment back to yard, new unit and equipment should come but with only
certified truck and driver with all requirements.
