Handover Tracker 13th July 2020

General

1. Emphasis on site changes around site and day to day operations due to COVID-19 must be
continuously carry on until it is adopted uniformly. Risk assessments should be thorough and
take into consideration the necessary precautions and risk. Foremen to ensure risk
assessments are done properly. Social distancing should be followed while carrying out work.
2. New maintenance plan has been sent out that outlines the 3-day configuration on
Wednesday, Thursday, and Friday for 6 hrs stoppages on each day. Please read carefully and
discuss with artisans, lock out procedure will have to be followed thoroughly as there will be
running equipment and standing equipment. This was done to limit amount of people onsite
and so that employees do not share accommodation while onsite. Weekly maintenance will
have to be flexible according to the work that must be done and resources available. EPCMM
vendor is being created, they need to be trialled and if happy with their standards then we
can use them, please follow up with extra people for this week. Site services and CMS to help
with plant work.
3. Planning must be held on Sun at 10AM and Tuesday 10AM, planning format amended to
include COVID-19 rules, supervisor, and non-permanent contractors and who will be
responsible for them. CMS, Consulmet, Metallurgist, Process Sup, Planner, Eng Foremen,
Safety Officer, Recovery Technician, Eng Manager or GES, Instrumentation FM/Tech,
Electrician must attend the planning meeting. Please make sure all jobs are over inspected
especially for lock out! Meeting must be conducted in English.
4. COVID-19 Procedures will be emailed to supervisors and foremen and should be discussed
with the teams for compliance.
5. Fogging as outlined in the sanitization procedure, dedicate one artisan a day to Fog all areas
in plant, maybe good job for nightshift artisan.
6. Foremen to close out safety deviations from daily safety officer report (All) – All contractors
are the responsibility of the person hosting them. The reports was emailed to everyone as
there are many closeouts needed especially for Consulmet.
7. Foremen must personally check safety files and make sure it has been approved prior to the
job.
8. Foreman must not sign any hot work, confined space, work at heights, excavation permits,
without first checking that the risk assessments are properly done, and lockout procedure is
included and correctly followed.
9. Costs must be managed as we are in difficult markets and we need to keep costs down.
10. Foreman must not sign service reports before receiving job cards from contractors.
11. Emphasize on housekeeping, spillage plan has been drawn up and must be followed!
12. Our Utilisation targets for 2020 financial year is 85% and 83% on long shut months, we have
achieved 83% overall in 2019 so much focus is needed to consistently achieve this.
13. South African Contractors should all go through screening and PCR Tests and produce up to
date results before being granted go ahead to travel to site and access into the mine.
14. Preparations for the signage with the new names has started. The two big boards that show
the plant layout have been put up in the plant near the boardroom and at the entrance of the
boardroom. The incoming shift should familiarize with the new names and start with
preparations, actual installations of the signs will gradually start on the following shift, doing
the changes in sections and not in one go. This was started and stopped due to resources
been isolated. Attendance registers must be signed by all process and engineering for
explanation of new names as part of change management.
15. EMC Trainer will help train/refresh our Telehandler drivers (6 months) and PTOs will be
signed off by Engineering Manager until arrangements can be made for the Usual trainer to
cross the border. Only a few complete as the rest were in isolation.
16. Updated handover feedback must be individually sent to the planners latest on Saturday.
17. Rootcauses are the worst that I have seen in a long time, please give this attention, I don’t
want the immediate cause, I want the root cause.
18. Everyone must understand that they are paid to work 21 days a month and that they need to
assist everywhere possible as they are only work 16/17 days per month.

1. Arrange shortlist for Electrician and interviews. Please re-advertise.


2. Convert cyclone pumps and thickener underflow 10x8s to water flushed. Follow up on
delivery
3. Arrange Structural Engineer to come to site and give recommendations for Jaw, Apron, XRT
feeders, CV15, cyclone and CM pumps rigging arrangements. Survey done, awaiting
quotations from Pagel
4. Blast damage to conveyors and roofs must be repaired urgently, list sent out. 90% complete
CV 01, 05, 04, 15 and workshop to be completed
5. Spillage clean up to continue as per spillage plan
6. Follow up on delivery of spare Kaiser compressor drive.
7. Infrared reports from Quest must be closed out urgently.
8. Please do oil sampling – arrange with local company, as Quest will not be able to do.
9. Plan full installation of DMS launders liners – When will this be done? Boilermakers training
on plasma machine to be completed first, please get this done.
10. We need to make a floodbox for XRT discharge onto Tertiary C screen, job was cancelled,
please redo.
11. Install air conditioner in the XRT container. When will this be done? Mr. Aircon on site to do
the installation. Quote received but not processed.
12. Install aircon in planning boardroom – 24000 BTU – When will this be done?
13. Aircons to be serviced urgently.
14. Get quotes for year-long (2020) Compressor services from Kaeser and CAS. It will not be
easy to get the guys in for the services. Get maintenance checklists and find out if we have
enough spares to do the services ourselves. Order placed for spares.
15. What is the progress with the New Spare compressor for the plant compressor?
16. Please CC Adheesh and Mahao in your handover notes.
17. Bus in Maseru, needs mechanic from Bloemfontein Iveco, please arrange to take bus to
them if possible. Was arranged and cancelled due to border issues.
18. Complete installation of field isolators for sizing screen, dewatering screen, flowsort tailings
and hand sort tailings conveyor. Pumps and screens completed belts will be completed on
the 1st July – Done.
19. Springs for screens are at Deugro, Ralejoe is trying to organise a few from Liqhobong for now
to fix feed prep screen. 6 onsite, to be returned to Liqhobong once our 50 arrive.
20. Please go through Bucanneer and Krebbs spares thoroughly as stores are bringing lists with
incorrect inventories and part numbers.
21. Req must be done for EPCMM once vendor is created
22. Follow up M12x50 and M12x60 full threaded CSK bolts
23. Follow up on delivery of spare motors and KSB pumps

Herman/Khotsofalang

1. Q2 2020 understudy assessment to be completed. Paperwork done. Understudy to read and


sign
2. Please repair zero noise and old generator for spare, see Khotsofalangs email. Old oil bath
generator spares are redundant. Suppliers do not repair them anymore. Will investigate
whats wrong with zero noise and repair
3. MCC and jetpump areas must be cleaned up. Cleaned
4. Door inside gloveboxes must be fixed. Busy ordering new doors.
5. Perspex on XRT vibro feeders must be fixed Perspex has arrived onsite. Will continue with job

7. Organize critical spare for the incinerator. LS codes were created. Was ordered, awaiting
delivery. Has arrived
8. Please oversee Jet pump valves installation and Jet pump automation. The project was put on
hold by Jan Bronkhorst until contractor can provide drawings.
9. Please make sure new assistant is sent for flowsort training. In process of organising dates
and order. Cancelled due to Covid 19
10. Jobcards must be loaded on Onkey. Ongoing, sending info to planners to create job cards.
Ongoing

Sefate and Mpiti

1. Cast base for the new Hydrex dam. Dam could not be delivered due to positive Covid 19
cases at hydrex. Dustin will deliver this week, base to be compacted and new dam installed.
2. Daily jobcard to be completed for sugarbowl and bi-weekly for supervisor. – Jobcard
created- To start printing from Thursday
3. Daily readings to be taken from V-Notch
4. Marcel is sourcing quotes for roof lifelines, please follow up
5. Drawings for new septic tank to be obtained and materials ordered, quote has been
processed for structural engineer’s drawings – Drawings received and proposed to engage
contractor base on scope on work. Contractor (Mampodi) to submit quotation on the 10 th
Jan. Three quotes received, a request for purchase order to be raised.
6. Please ensure good site housekeeping, start getting rid of unnecessary containers and old
spares/scrap lying around. Plumber’s workshop roof to be removed.
7. Water operator interviews to be completed and person appointed – readvertise
8. Jobcards must be loaded on Onkey – Site services – Sewerage Treatment plant and Artisan

2. CAT 1and Perkins are down. Plan for base box programming on CAT 1 and repairs on the
diesel heaters on the Perkins. - Done

4. West Valley rental generator to be sent back - Done
5. HHK audit completed, please get final report and start actioning – In progress, no PVC
conduit- Awaiting Delivery of conduit
6. Recovery reticulation is not clear, and drawing must be completed, Recovery to be
prioritised. In Progress.

9. Transformer bays, bund walls and roofs to be constructed. MCC 3&4 transformers needs
urgent attention. In progress. Please get quotes for the roofs over the transformers. MCC

10. MCC 2 220V transformer to be installed. Please trace control circuit properly before

Willem

1. CV03 belt scale to be repaired - done


2. Please get schedule and drawings for Jet pump automisation. Shipped to site. Please manage
the installation and commissioning closely. Drawings will be submitted by Jan Bronkhorst,
work delayed due to the passing of Ferdi and will commence early Feb. Put on hold until
Consulmet advises
3. Gamma check inspection to be done, please get report, items on report to be closed out
urgently. – Orders placed for spares
4. Slipping device to be installed on hand sort 2 belt. Only when Jet pump automisation is
completed- on hold
5. Training to be arranged for radiation officers. – Send me certificates
6. Scrubber drive running amps (current) to be displayed on SCADA, check for output on digital
ammeters.
7. Commission remaining belt slip devices – when? Three still to be installed.
8. Check if third process pump behind blue tank for secondary screen can be started from the
SCADA. Pump bypassed while awaiting program manipulations.

Planners

1. Complete DVM
2. Make sure Downtime dashboard downtime are correctly allocated and should be same as
maintenance KPI’s, updated and is been checked from time to time…… ongoing.
3. Planners must make sure root causes are filled in and filed, check all root causes for the
month, RCA document not filled culturally, both Process and Engineering Foreman’s to assist
drive the process, ongoing.
4. Earth leakage Jobcards to be updated according to I-Control reports
5. Please get other reports functional in Onkey asap especially artisan utilisation. New module
loaded which may give all reports needed, Marcel to go for training from the 24 th Feb where
he will learn how to use this new module. Mock-up reports to be send to Onkey as a sample
of required report structure as well.
6. Send incorrect gearboxes back to Bonfiglioli and get credit – 4 gearboxes left to order, spoke
to Quintin to swop 5 that is not needed – Done. Are they converting them free of cost?
Where is the quote for 3 gearboxes sent?
7. Marcel to finish up bus business case - 85%, please get updated quotes. Follow up Ashwin for
bus quotes.
8. All shift Jobcards to be returned/received before going off shift – 100% Jobcard attainment
compulsory
9. Post shut meeting should be held on Thursdays and minutes taken and distributed to all
stakeholders including the off shift.
10. Marcel to order XRT fixed high pressure washer – 550V, ordered. Follow up on delivery

Mahao/Adheesh

1. Arrange meeting for Sandvik to discuss way forward with Ter C as gear and pinion was
changed but noise and heat is still there
2. Familiarise employees for new conveyor names – Get attendance registers from process and
engineering
3. Arrange weights for tertiary MVT screen – MVT is already machining weights for the screen,
they will confirm completion and deliver to site. - Installed
4. DVM must be done weekly and monthly
5. Procurement meeting to be held on Thursdays
6. Fix dashboard graphs – still not satisfactory – Foremen need to improve asap.
7. Decision on backup plant air compressor to be made – repair old or buy new (upgraded size)
8. Follow up on Apron Modification – modified plummer block adjusting blocks delivered to site.
Installation awaiting two-day shut. Install on Wednesday with new belt.
9. Radiation Officer appointment – Pauline to submit appointment for sign off
10. Arrange procurement meeting to follow up on spares – Done for the past week
11. Follow up of LT125 pecker repairs – awaiting a cable, the delivery to site expected this week,
cable delivered but has wrong plug, build catladder and platform or investigate moving
controls
12. Revise lifting and Rigging procedure
13. Make sure all expats travelling to site have did PCR tests in advance and that the site
manager has been made aware, see email sent to you

Consulmet

1. Metso LT120 and LT125 operators, submit PTOS


2. Please make sure the Structural integrity project is running smoothly and safely – Lots of
safety deviations, keep monitoring as high-risk work -o
3. Both tertiary and secondary liners to be inspected weekly and crushing edges measurements
to be recorded – Ongoing
4. Plan for replacing buffers on the sizing screen in recovery.
5. Secondary A liners was changed on the 20 November with 442.8814 and 442.8820 and due
on 20 May
6. Secondary B liners was changed on the 26th March and started on 7 th June, keep monitoring
and is due on the 7th of November
7. Tertiary A was changed on the weekend of the 11TH June 2020 and put into production on the
12th June 2020, Sandvik liners used here – inspect every maintenance
8. Tertiary B was changed on the 4th July 2020 and it ready to use.
9. Tertiary C was changed on the 14th June 2020, not running properly, we will arrange another
meeting with Sandvik as noise was still there even after gear and pinion was replaced.
10. Use Rema tip top to splice LT125 belt as Pilot crushtec was going to subcontract them in any
case.
11. Get Job cards for 1000 hrs service from both METSO crushers from planners and carry out
services ourselves 18th and 19th June 2020 respectively - Complete
12. Crane rope and sheave to be replaced and approved by Terex – Complete, Consulmet to send
new cerificates
13. Arrange weights for MVT Tertiary screen - Complete
14. Check Sandvik and Metso spares – See email
15. All grating to be audited as part of close out for Ter A incident and blank off to be fabricated
for secondary and tertiary pan feeders.

17. Follow up on scrubber wheel repairs and scrubber gearbox repairs, was sent away.
18. Follow up on Ter C oil filters delivery for the main lube line.
