External Service - Scope of Work/Terms and Conditions

Safety Procedures
1. Review JPT and create LST with supervisor
2. Wear mandatory PPE (hard hat, protection goggles, high visibility clothes, safety shoes and additional PPE
applicable in the working area (dust mask, safety gloves, ear plug, thermal suit).
3. Follow standard plant safety procedures.
4. Report any unsafe conditions that may affect personnel safety to the area owner/ champion.
5. Install padlock and tag.
6. Conduct local test in the field to make sure the equipment is isolated

Job Steps
1. Fabricate grit cone worn out parts
-Measure and cut plate for re-plating worn out parts
-Patch and weld cracks and worn out portion, full weld plate
2.Patching/re-plating of VRM feed chute lower portion
-Measure and cut plate to be used for re-plating
-Trim down/cut worn out portion and install plate for re-plating
-Full weld plate and grind off excess welding deposits until surface is smooth
-To the same process on other worn out areas of chute
3. Fabricate & install dam ring
-Measure dam ring height and cut plate/bar to be used for restoration of dam ring height
4.Tripple gate patching of worn out damper and casing
-Measure and prepare plate to be used for patching
-Cut out worn out portion and install plate to be used for re-plating
-Full weld plate and grind off excess welding deposits
5.Correction of false air at VRM area, mill body ducting
-Erect scaffolding under areas with mill body holes
-Weld holes and grind excess deposit
-Patch big holes and full weld plate, grind off excess welding deposit

Terms and Conditions:


1. Contractor to supply labor, tools / equipment and supervision to complete the job
2. Proponent to provide area for temporary facilities, power supply (at tapping points only) for the contractor
3. Contractor will be responsible for the safety of his personnel in the work area. Proper identification cards
coterminous with the project duration shall be provided to all of the Contractor's personnel. All necessary
safety signboards and warning signs are to be provided at the project site throughout the duration of the
project. All Contractors' workers shall be provided with proper PPE's.
4. Contractor shall be held liable for all damages to persons, materials and equipment that might occur as a
result of the contractor's negligence during the execution of the contracted work. In case of accident,
Contractor to submit within 24 hours, a written accident report to the Project Proponent
5. Contractor shall coordinate at all times with the Project Proponent / Owner's duly authorized representative
during the execution of the contracted work
6. Defects in materials and workmanship found during turnover/final inspection are to be rectified by the
Contractor at no extra cost to the Owner.
7. Contractor must verify actual project site condition and shall include in their offer items of work not shown
nor stated but are deemed necessary to complete the project.
8. No work shall be covered-up nor place out of view without the Owner's approval. Contractor must give full
access and opportunity to Owner to examine / measure any work which is about to be covered-up or placed
out of view or before permanent works are placed thereon.
9. Contractor to implement housekeeping every end of the shift at the project site. Should the Contractor failed
to implement daily housekeeping, Owner may facilitate the cleaning of the area by hiring another entity at a
cost chargeable to the Contractor's account without requiring the Owner of any proof of the cost for such
deduction.
10. Owner reserves the right to make changes in the plans and specifications as the work progresses to suit
actual conditions.
11. Contractor must process necessary documents (i.e. Barangay clearance, task analysis, works permits etc...)
before the start of the project.
12. Contractor and its personnel must undergo safety briefing prior to the start of the project. "No safety
briefing No work".
13. Tool box meeting MUST be conducted by the contractor before every start of the shift including safety.
Document and submit to the proponent every end of the shift.
14. All contractor personnel must sign the action log pertaining toolbox and safety issues.
15. Contractor to fill-up the supervisor's diary and discuss concerns with the area champion every end of the
shift or immediately if the findings needs actions.
16. Contractor to submit list of personnel with corresponding skills and designations prior to the start of the
project.
17. Contractor to close the safety work permit and sign the turn-over document to the proponent before
leaving the plant.
18. Contractor to update and submit gannt chart daily.
19. Within seven days after the work, the invoice should be given to project proponent for service entry
