4. COUNSELLING ENGAGEMENTS:
 SWORN: Includes Regular Police and District Constables
 UNSWORN: Includes all civilian personnel i.e. Office, ancillary and grounds who work in the J.C.F.
 OTHERS: includes all other persons who do not work in the J.C.F.

5. ENGAGEMENTS should be reported on ONLY if you participated as a Peer Counsellor.

6. REMARKS: Record any outstanding achievement(s) and describe any problem (s) you may be having in your role
as a Peer Counsellor. This should only relate to the mission of the Chaplaincy Services Branch.

7. SUBMISSION OF REPORT: Peer Counsellorsmust submit a copy of this report to the Assistant Chaplain and
his/hersecretary no later than three(3) days in the following month. The respective Chaplaincy units should submit
a copy of this report to the secretary at the Chaplaincy Headquarters no later than one (1) day after receiving
report.
