Dear Sir/Madam:
In his/her stay with OFW , _______________ is directly employed by Saint Michael Manpower and
General Services who shall be responsible for his/her salaries/wages/benefits as provided for by law.
He/She will be under our control and supervision and shall not perform duties of your regular employees.
________________ will be assigned to one of your Outlets/Branches at OFW starting, JUNE 15, 2019.
He/She will abide/follow the rules of our Agency, which are aligned with the general rules and
regulations set forth by your company.
Likewise, in cases where she/he commits/violate any of the company rules of OFW , you as our client,
may coordinate with us, so we can take action and impose proper disciplinary action. While he is being
investigated, we guarantee to provide a replacement for him. In case of pilferage and other crimes that
may result to damage to your company and if proven guilty during investigation, our company has the
sole responsibility to return and pay off/charge the loss incurred as a result of the pilferage investigation
and /or to any similar incidents such as mentioned above.
It will be highly appreciated if your office will notify us immediately regarding any in traction or
violation committed by our employee.
