AFFIDAVIT OF TOTAL LANDHOLDINGS

I, ABC, of legal age, widow, Filipino and a resident of Matina Davao City, after
being sworn to in accordance with law, do hereby depose and state:

1. I am the widow and heir of the late XYZ, who died intestate, last May 17,
2014, without any will;

2. My late wife was the registered owner of a parcel of land covered by

Transfer Certificate of Title No. T-12344, situated in Barrio Matina Davao City, Davao
del Sur, Island of Mindanao, Philippines, containing an area of TWO
HUNDRED SIXTEEN (216) SQUARE METERS, more or less;

3. The total landholding of my late wife and owned conjugally by me is the

only parcel of land stated in the previous paragraph hereof;

4. I execute this affidavit to attest to the truth of the foregoing facts and to
apprise the proper authorities thereof.

IN WITNESS WHEREOF, I have hereunto set my hand this 16th day of
