Student’s Action or Behavior: Justin was playing along with his classmates during lunch
time. While he was happily sliding on the stone, he slipped and accidentally dropped
by the stone and was wounded on his hand. Justin was immediately run by his
accompanied friends to the school clinic with Ma’am Paulette Tan our School Nurse
who was their giving him first aid. When I found out what was happened I
immediately take action, after the first aid Ma’am Tan told me that the wound was
deep and should be injected with anti tetanus. I immediately called the parents of
Justin to let them know the situation and be taken an immediate action. At the end,
Justin was okay and was given an antidote for his wounds.

Evaluator’s Interpretation:
Children should NOT be allowed to play along the restricted areas prone to accidents
especially those areas with big stones that might harm and be cause of accidents. They
should be reminded the possible accidents that might happen if they will continue
playing on those areas. And as a teacher I should be more careful and watchful to the
kids by giving them constant reminders as of the do’s and don’ts inside ad outside the
classroom.
