JOINT AFFIDAVIT

WE, Benito A. Palmero, Jr. and Ruel A. Nael of legal ages, both married and residents of
Barangay San Roque and Barangay Pambuhan, Mercedes, Camarines Norte respectively, after having
been duly sworn on oath in accordance with law hereby voluntary depose and say:

1. That we three (3) Municipal Councilors of the Sangguniang Bayan of Mercedes, Camarines
Norte namely Councilors Benito A. Palmero, Jr. and Ruel A. Nael together with Councilor
Francisco L. Ramos, Jr. have attended the 2 days seminar on “Eco Solid Waste Management
Integrating Rapid Damage Assessment and needs
Analysis Towards Climate Change Mitigation held at Golden Dragon Restaurant Hall, Subic
International Hotel, Subic Freeport Zone, Olongapo City on March 28-29, 2019
Sponsored by Eco-Forum Unlimited;
2. That we three (3) Councilors were issued check as cash advances intended for the said 2-days
seminar which include our Registration Fee, Bus Fare and Per Diems in Olongapo City;
3. That we three (3) Municipal Councilors Palmero, Nael and Ramos rode at Philtranco Bus
going to Pasay City on March 26, 2019 at around 9:30PM and reached at 6AM in Manila; and
ride again another Bus going to Subic, Olongapo City on March 27, 2019 at 7AM and reached
Olongapo City at around 10:00AM;
4. That other two (2) members of the Sangguniang Bayan namely Municipal Councilor
Jesus King and ABC President Ex-Officio in the Sangguniang Bayan of Mercedes Garry Patrick
SP. Cootauco was also in attendance of the said seminar together with other Barangay
Councilsof Mercedes, Camarines Norte including SK Chairpersons of Mercedes, Camarines
Norte;
5. That we the three (3) councilors were billeted in the Subic International Hotel together with
all the members of the Barangay Council of Barangay Mambungalon;
6. That on May 27, 2019, we five (5) councilors simultaneously liquidated our cash advances for
the said travel dated March 28-29, 2019 including our bus tickets, appearances and payments
for our registration fees and the required terminal report for the said seminar;
7. That we are executing this statement to attest to all the above facts.

IN TRUTH WHEREOF, we have hereunto signed our names below, this 20 th day of November,
