I. COMMUNITY BACKGROUND
Provides the condition and profile of the people living in that
community. It explains the issues and problems in the community

II. TARGET RECIPIENTS


Describe the specific groups or individual who will receive/benefit
the project.

III. DESCRIPTIONS OF THE PROJECT


Is the title and the nature of the project. It is the main focus of the
group to undertake within the given time.

IV. OBJECTIVES
Explicitly stating the goals of the project and the indicators of
conclusion; maybe broken down into general and specific
objectives. (Objectives should be SMART) Simple or Specific;
Measurable; Attainable; Realistic; & Time Bounded.

V. SCOPE AND LIMITATION OF THE PROJECT


Identifies the limitation of the project; how will you know that the
proj. has already been concluded. It includes the brief statement
of the general purpose of the project and the population.

VI . SIGNIFICANCE OF THE PROJECT


Contains the significant implications of the project to the target
recipients, Barangay, group, school and the like.

VII. METHODOLOGY
This includes a detailed description of all the materials and
methods that will be used in course of the implementation of the
project. Procedures should be stated in as much detailed as
possible so that readers can easily perform them as they read your
work.

VIII. SCHEDULE OF THE ACTIVITIES


Refer to the series of activities with expected output, persons
responsible, time frame and resources needed.

IX. RESULTS
Covers the actual events as the results of the plotted schedule of activities. Results
should be shown from day 1 to the last day of project implementation. Results may not
be exactly the same with the proposed schedule of activities.

X. SUMMARY
Refers to the nutshell of the project

XI. CONCLUSIONS AND RECOMMENDATIONS


This can be done in paragraph form or bulleted. It includes the
impact of the project implemented.

XII. INDIVIDUAL OUTPUT/Learning


Refers to the insights of each member/Individual learning.
Individual output shall be done strictly in one page per member.

XIII. APENDECIES & EXHIBITS


It includes but not limited to pictures, videos, certificates, receipts,
accounting/financial report and other reports and documents
related to the projects undertaken by the group.

Technical Requirements:
. 1 ½ spacing
. Font size : 12 maximum
. Font style : Arial
. Long Bond Paper
. Sliding folder for the proposal/Clear book for the final doc.
. Final document should not be less than 20 pages excluding appendices & exhibits
Final documents shall be submitted 3 days before the actual defense schedule (Schedule of defense is
announced during the 11th week of implementation).
