Please confirm following before we can offer you job with our clients

1. You are fully “FIT TO WORK” in your opinion, and you do not have any hidden fitness
problem which you have not disclosed to DG Doctor.
2. You do not have any family problems or health issues of your any family members,i.e.
Mother/ Father/ Wife / Children which require your presence or attention.
3. You have someone at home who can attend to any family problems or ill health of any of
your family member in your absence while you are on ship.
4. If you are married, then it is assumed that your wife is not expecting within 6 months.
5. Whatever salary will be offered to you from our clients will be confidential information and
you are to assure that will keep that confidential and not to disclose to any other employee
except Master.
6. If required for early Sign Off due to any of the above reasons, then the total repatriation
cost of you and your reliever will be deducted from your wages.
7. I am happy with my current salary.
8. I am responsible for all my documents (including flag endorsements) and shall carry them
always with me and not leave them on Vessel.
9. Repatriation Cost will also be borne by you in case of the following situation:
  If you fail to perform your duties
 If you were found under influence of Alcohol or Drug
 If you lead to Sign off to attend any Urgent Work. [Emergency may be exception
(Health, Sickness) but solely the description of the Company]
