Parental Consent

I, ________________________________, the parent/guardian

of _______________________, allow him/her to join the “Mr.
and Ms. Timber City Academy 2020”, an annual pageant that
attempts to see the exemplars of beauty, wit and character who best
represents the institution dating from January 31- February 13,
2020.
This is also to prove my knowledge that my son/daughter will
engage into various activities before, during and even after the
competition.
Furthermore, I am also aware that expenses either personal or
involving multiple parties are already agreed upon by my behalf
and of the organizers.
Lastly, I acknowledge that the school will never be held liable
for whatever undertakings my child will do prior and after
schedule stipulated unless formally accounted to school’s
representatives/organizers.
