AFIDAVIT OF LOSS

I, BILLY JOE RESABA y MOJADO, of legal age, Filipino, single, and with
residence at Purok 4, Brgy. Bulacnin, Lipa City, after having been sworn to in
accordance with law, hereby depose and state:

1. That, I am a holder of a payroll account at Rizal Commercial Banking

Corporation (RCBC) for which I was issued an Automated Teller Machine
(ATM) card;

2. That, I usually keep such ATM card in my wallet which I habitually place
inside my bag;

3. That, sometime in August 2016 when I was going to pay for something I
bought I could not find my wallet where I keep my ATM card;

4. That, I exerted effort to locate my wallet where I kept my ATM card but it
could not be found and my ATM card appears now to be beyond recovery;

5. That, I execute this affidavit to attest to the truthfulness of the foregoing and
for whatever legal purpose it may serve.
