SUBJECT : Spot Report on Vehicular Incident

1. What : Report on Self Crash.

2. Where : Provincial Road Sitio Cabar, Brgy. Aribungos, Brooke’s Point,

Palawan. Latitude 8.91898. Longitude 117.826432.

3. Who : Alvin Baldevino Asan, 20 years old, born on July 28, 1998,
born at Brooke’s Point Palawan and residing at Sitio Cabar, Brgy. Aribungos,
Brooke’s Point, Palawan with back rider Dindo Tibak Inso 22 years old, born on July
15, 1997, born at Brooke’s Point Palawan, and residing at Sitio Cabar, Brgy.
Aribungos, Brooke’s Point, Palawan.

4. When : June 4, 2019 at about 6:35 PM.

5. How : This pertain on the report recieved by this station from a

concern citezen informing that Vehicular incident transpired at the aboved
aforementiond placed, Immediately personnel of this Station led by PEMS Jose V
Lumilay and two PNCO responded at the area of incident to verify the veracity of the
report and conduct investigation. Investigation disclosed that one Alvin Baldevino
Asan was driving his Honda TMX125 color Black motorcycle with his back rider and
traversing at provincial road of Bgy. Aribungos, this municipality running on high
speed upon reaching at Sitio Cabar, said driver lost control and went out off the road
and suddenly bumped on motorcycle, parking at the side of the road, said driver and
back rider slumped at the pavement of the road and sustain serious head injuries,
immediately both driver and backrider rush to SPPH for medical treatment but back
rider pronounce dead by attending physician. Accordingly said driver and back rider
was from a drinking session and driving under the influence of intoxication liquor.
