    damage to the water lines located at Purok 3, Umangan, Aliaga,
    Nueva Ecija. This is a result from Excavation of your line in that
    location wherein water pipes are unfortunately ruptured.
    The following are the materials used and labor exerted for the repair
    of the said damages.
