This form is to serve as a permanent record of a student teacher's professional performance evaluation during a specific time
period based on specific criteria. This form must be used at least twice during the 12-week (minimum) student teaching
experience.
PERFORMANCE EVALUATION
Directions: Examine all sources of evidence provided by the student teacher and bear in mind the aspects of teaching for each
of the four categories used in this form. Check the appropriate aspects of student teaching, and indicate the sources of
evidence used to determine the evaluation of the results in each category. Assign an evaluation for each of the four categories
and then assign an overall evaluation of performance. Sign the form and gain the signature of the student teacher.
Criteria for Rating Categories
Exemplary: The candidate consistently and thoroughly demonstrates indicators of performance. 3 Points
Superior: The candidate usually and extensively demonstrates indicators of performance. 2 Points
Satisfactory: The candidate sometimes and adequately demonstrates indicators of performance. 1 Point
Unsatisfactory: The candidate rarely or never and inappropriately or superficially demonstrates indicators of performance. 0 Points
Category I: Planning and Preparation - Student teacher demonstrates thorough knowledge of content and pedagogical skills in planning and
preparation, student teacher makes plans and sets goals based on the content to be taught/learned, their knowledge of assigned students and their
instructional context.

Justification for Evaluation:

Miss Nye has excellent understanding of content, pedagogy, and the Pennsylvania Academic Standards. She makes it
a priority to know her students and delivers instruction that appeals to her students' strengths and weaknesses. She is
able to adapt curriculum to meet individual student needs and uses both formative and summative assessments to
guide her instruction. Melissa employs a wide variety of resources including materials she has created herself. She is
conscientious about aligning instruction with content goals and works to scaffold student learning. Melissa
challenges her students but has supports in place that help the students reach success without facing frustrations.

Justification for Evaluation:

Melissa has invested a great deal of time into getting to know her students, This has included examining records,
assessments, IEPs, etc. She maximizes the use of downtime to build relationships. She asks questions of her
cooperating teacher and does not hesitate to seek out help. Melissa has successfully taken over the full load of
teaching and all that this entails including grading papers, keeping accurate records, behavior management, etc. She
provides opportunities for all students to learn, making adaptations as needed. Melissa incorporates flexibility into
the classroom routine, but also is cognizant of time and its importance. She has a fun sense of humor which her
students enjoy immensely. Finally, Melissa treats all students equally and fairly.

Justification for Evaluation:

Miss Nye motivates students to perform at a high level of achievement. She establishes high, yet achievable goals for
her students. Melissa is very skilled at questioning strategies. She can readily scaffold questions to take students from
their existing knowledge to a higher level. I enjoy observing her carry out this process and am always amazed when
students successfully reach a new level because of Melissa's patience and structured questions. Melissa also creates a
lot of materials that provide ways for students to actively engage in their learning. Using both formative and
summative assessments, these activities are always well thoughtout and align with the objectives being taught. These
activities are successful because Melissa has taken the time to know her students. She shares multiple ways for
students to solve problems and always models activities. She understands the instructional technique of "I do, We do,
You do!" Miss Nye also provides opportunities for students to collaborate and discuss ideas and details. She uses
anchor charts and visuals that provide excellent references and resources for the students to use and help them
become independent learners. Melissa also employs excellent transitions from one topic of learning to the next thus
making learning seamless. She has incorporated many behavior management strategies into the classroom. Students
are motivated and disruptions are rare. Miss Nye frequently offers specific praise when students complete a task or
advance a skill. There is always a warm, wonderful environment present in her classroom.

Justification for Evaluation:

Miss Nye possesses the skills of an effective teacher. She understands the commitment involved in becoming a
teacher and is dedicated to the profession. Melissa puts in long hours creating materials and developing sound
activities for her students. She motivates her students and recognizes the need to provide students with various
activities to appeal to different learning styles. Creativity and organization are two of her strengths that serve her
well. Melissa does an outstanding job at connecting learning from one activity to the next so that students aren't just
learning content in isolation. Melissa assures that physical needs of the children are met and works to promote
healthy mental attitudes as well. Miss Nye provides an environment where students are safe, excited to learn, and are
cherished for their individuality. Students are respected and respectful.

Additionally, Melissa has knowledge of the school district's rules and policies. She arrives to school early so that she
can interact with her students when they arrive to school. Miss Nye is one to stay after the school day's end to finish
preparations for the following school day. She attends conferences and faculty meetings. Melissa is a natural
collaborator. She works well with the faculty and the staff to promote quality instruction and student learning.

Finally, Miss Nye is a person of integrity and consistently displays high moral values. She is a wonderful role model
for children and adults!

Justification for Overall Rating:

Melissa has been very successful in this placement. I am excited to see what first grade offers her and how she responds. I am
confident that Miss Nye will continue to be sucessful.
