FRONT LINE COUNTERS

REQUEST:

 The approval of emergency purchase and installation of acrylic panels for

frontline counters.

JUSTIFICATION:

 The panel in the NAC, EA and Document Examiner counters will serve as
protective barrier from exposure to corona virus since the above-mentioned
employees directly interact and engage walk in clients,
 This is to supplement with the other safety procedures and existing health
protocols being observed by the branch considering that there are covid
cases that are asymptomatic and/or at the early stage of the disease which
may not be detected by mere forehead thermometer,
 At a minimal cost difference, the panel will preserve and maintain the bank’s
corporate image compared to ordinary cellophane susceptible to tearing. The
estimated cost of the installation of acrylic panel is at P 35,220.00.
 A panel made of acrylic is more durable and can withstand more than
ordinary wear and tear.
 This is also in compliance to EO No. 50 Series 2020 on Workplace Security and
Safety.

SUBJECT : EMERGENCY PURCHASE REPAIR OF GENSET

DATE : June 04, 2019

REQUEST:
Emergency purchase of defective parts and repair of Generator Set

BACKGROUND:
 During the conduct of regular testing of generator set on May 28, 2019, it
was observed that the voltage is unstable and fluctuates from 225v to 99v.
Then an error message “Common Alarm” is displayed.
 The unit has undergone intensive checking and troubleshooting by technician
from the LS Diesel Starter Parts & Engineering Services Davao on June 03,
2019 and found the ff problems:
- Defective oil sender
- Busted AVR
- Low insulation resistance for the stator and rotor
 Accordingly, if the detected problems are resolved, the genset will resume to
its normal operation.

JUSTIFICATION:
 Repair and replacement of defective parts is immediate and necessary so the
branch operation will still continue during power interruption.
 Attached is the quotation from LS Diesel Starter Parts & Engineering Services
from Davao City. No local technician is available to perform the repair needed
for the unit.

REQUEST:
Emergency check-up of Dieselec Generator Set

BACKGROUND:
 During the conduct of regular testing of generator set on May 28, 2019, it
was observed that the voltage is unstable and fluctuates from 225v to 99v.
Then an error message “Common Alarm” is displayed.
 Request for checking/troubleshooting is requested at once to determine and
correct the cause of this error and restore the good performance of the unit.

JUSTIFICATION:
 Immediate check up by a qualified technician is necessary in order to correct
error and avoid disruption of branch services during power outage.
 Attached is the quotation from LS Diesel Starter Parts & Engineering Services
from Davao City. No local technician is available to perform the trouble
shooting.

SUBJECT : HAZARDUOUS WASTE STORAGE

DATE : May 20, 2019

REQUEST:
Request for emergency construction of hazardous waste storage

BACKGROUND:
 During the conduct of EMS Briefing last May 18, 2019, GPCO Madison
Ballaga, noted that the branch existing storage area is not big enough to
accommodate all the wastes generated by all the units (Branch, PVCID, LC
and AC) housed in this building.
 Since there is no regular schedule for disposal of waste by accredited
transporter of DENR and with the impending 3rd party IMS Audit, there is an
immediate need for this request.

JUSTIFICATION:
 Branch PCO can better manage, properly contain and safeguard human
health and environment with a bigger storage area.
 Cost for labor and materials is estimated at P15,000.00; to be allocated
proportionately by all the units housed in this building.
