AFFIDAVIT OF LOSS

Gutierrez Comp. Paso St., Bagumbayan, Taguig City, after having been duly sworn to in
accordance with law, and in behalf of my minor son ANTONIO A. RONDEZ IV,
hereby depose and say that:

1. Sometime around December 2019, while going home from school, my son,
Antonio Rondez IV, accidentally dropped his school I.D. on the street;

2. The school I.D. was for the current school year (2019-2020);

3. We exerted diligent efforts to locate the school I.D. but to no avail and I therefore
considered the same as lost and can no longer be found;

4. I am executing this Affidavit to attest to the truth of the foregoing facts and to use
the same for whatever legal purpose it may serve.
