I. Objectives
Identify the tips in creating survey forms
Create a printed survey form
B. Motivation
The teacher will let the students form a picture puzzle on the board of
an online survey form. The teacher will ask the pupils what is the puzzle that
they formed. (see LM attachment survey form tarpapel)
C. Unlocking of Difficulties
Survey
- comes from the Greek word surveier, from medieval Latin
supervidere – super means “over” and videre which means
“to see.”
- is a method of gathering information through a brief interview
or discussion with individuals about a specific topic.
D. Presentation
Based on the picture puzzle formed, what are the uses of a survey
form?
The teacher will enumerate the tips on how to create a survey form and
the students will identify if it followed the Jollibee sample survey form. (see
LM attachment for the PPT)
E. Deepening
The teacher will group the students into five.
The group will be given one sample survey form which they will critique
if it followed the tips on how to make one. (see LM for sample survey forms)
F. Generalization
What are the tips on how to create a survey form?
G. Application
The pupils will create a printed survey form using pen and paper. (see
attachment for the rubrics)
