uses to request the purchase of a specific item.
2. Procedures in filling up a requisition form:
a. State the name of the project and date when you requested
for materials.
b. Fill in the classification of the project and its purpose.
c. Write the number of required materials.
d. List the unit of materials to be requisitioned.
e. Enumerate the materials with their description.
f. List the cost per unit and the total cost.
g. Write your name as requisitioner.
h. Secure the approval of authorities concerned
