IV Procedures
A. Introduction Activity No. 1 - IDENTIFY ME!
Direction: State whether each relation y = f(x)
describes a quadratic function or not.
1. y = x² - 5x + 2
2. y = (x +2)(x- 3)
3. y = 3x + 3
4. y = 2( x + 5)² + 7
5. y = x³ + 4x² - x – 2
6. y = 2x – 10
7. y = (x – 2) (x + 4)
8. y = (x – 3)2 + 1
9. y = 3x2 +4x – 1
10. y = 2x -3x – 2
B. Development A. From Activity No. 1, answer the following
questions:
1. How many are quadratic functions?
2. Group the quadratic functions according to
their similarities.
3. How many groups were formed?
4. What have you noticed on each group why
they (quadratic functions) belong with each
other?
5. What do you call each group?

B. Activity No.2: WHO AM I?

Direction: Guess a word or words according to the

given clue/hint?

1. What is the gerund form of transform?

ANSWER: Transforming
2. What do you call an army officer of a very
high rank?
ANSWER: General
3. Complete the sentence: Coal is a ____ of
carbon.
ANSWER: Form
4. What preposition is used to indicate reference
or to specify a number or amount?
ANSWER: Of
5. What do you call a polynomial of the second-
degree function?
ANSWER: Quadratic Function
6. Complete again the sentence: The children
jumped _____ the lake for a swim.
ANSWER: Into
7. What S has something set up and established
by authority as a rule for the measure of
quantity, weight, extent, value, or quality?
ANSWER: Standard
8. What verb is used to make something into a
particular shape?
ANSWER: Form

C. Transform y = x2 +6x +8 into the form y = a( x

– h)2 + k by following the steps below.
(completing the square)
1. Group the terms containing x
2. Factor out a.
3. Complete the expression in parenthesis to
make it a perfect square trinomial.
4. Express the perfect square trinomial as
square of binomial.
5. Give the value of h
6. Give the value of k.

D. Transform y = 3x2 – 4x + 1 into standard form

by using the formula of h and k.

b 4 ac−b2
h= - 2 a , k=
2a

Then, substitute it in y = a(x-h)2 + k

C. Engagement Answer the following exercises:
1. Activity No. 9, LM’s page 134 – 135
2. Activity No. 10, LM’s page 135 – 136
3. Worksheet 1
4. Worksheet 2
5. Worksheet 3
D. Assimilation Quadratic function is a polynomial of second-degree
function.

The general form of quadratic function is in the

form y = ax2 + bx + c, where a, b, and c are real
numbers and a≠0.

The standard form of quadratic function is in the

form y = a(x – h) 2 + k also known as vertex form.

To transform the general form y = ax 2 + bx + c of

quadratic functions into standard form y = a ( x – h) 2 +
k:

A. Using the completing the square

Steps:
1. Group the terms containing x
2. Factor out a.
3. Complete the expression in parenthesis to
make it a perfect square trinomial.
4. Express the perfect square trinomial as
square of binomial.
B. Using the h and k formula
Steps:
1. Find the value of a
2. Find the value of h
b
h= - 2 a ,
3. Find the value of k
4 ac−b2
k=
2a
4. Substitute the three values in
Y = a(x – h)2 + k

VI Reflection The topic transforming general form of quadratic

functions into standard form is quite difficult to
understand by the learners especially if they lack
knowledge on completing the squares so I used
different approaches such blended learning approach
and others.

I realized that teaching this topic is very essential to

the learners for them to apply it in real life situation.
(Example: If problem/s occur in our life don’t give up.
Set up plans. Plan A and Plan B. For every problem
there is solution.)
