AFFIDAVIT OF EXPLANATION

I, JANICE B. NOVILLAS, of legal age, married, Filipino and a resident of

Brgy. Ungka I, Pavia, Iloilo, Philippines, after having been duly sworn to in
accordance with law, depose and say:

1. That I am a bonfide holder of one (1) Philippine Passport issued by

Department of Foreign Affairs - Iloilo;

2. That the afore-mentioned Passport was lost, which prompted me to execute

an Affidavit of Loss dated October 14, 2019 duly notarized on the same date
by Notary Public Kazper Vic Bermejo as per Doc. No. 14, Page No. 4, Book
No. 1, Series of 2019. Copoy of the said Affidavit of Loss is hereto attached
as Annex “A”.

3. That by reason of the loss of my Philippine Passport, I have no choice but to

re-apply for its replacement in lieu of the loss one;

4. That at the time I was re-applying for its replacement, I found out that my
husband was keeping a paramour;

5. That by reason of that discovery and remorse, I inadvertently wrote in my re-

application for replacement of my Philippine Passport, specifically in my civil
status “single” instead of “married;

6. That I have no intention to mislead the Department of Foreign Affairs - Iloilo

in relation to my civil status.
