may involve taking notes.

database in form of assessment outputs.

7. I have been informed that the data acquired from me by the student would not
require any specific confidentiality measures to be taken.

8. I understand that given the small number of participants involved in the study, it
may not be possible to guarantee my anonymity.

9. I understand that after I sign and return this consent form, it will be retained by
the university and the student.

10. I understand that I am willingly participating in the data collection for this
academic activity.
